Minimum Qualifications MINIMUM QUALIFICATIONS:

Currently a permanent Rehabilitation Counselor 2 or 70.1 Transfer Eligible within two grades as determined by Civil Service.

Promotion Qualifications: Simultaneous to this posting, the civil service list for Rehabilitation Counselor 2 may be canvassed. Duties Description PRIMARY DUTIES AND RESPONSIBILITIES:

• Provides all levels of monitoring and assistance to state operated programs in the area of Incident Reporting, including review, monitoring and oversight of incidents. This would include, entering tracking of incident data in spreadsheets, conducting IRC Committee meetings, ensuring minutes and agenda are completed and distributed timely, monitoring entries in IRMA, tracking incident follow-up and protections to ensure recommendations are completed and processed timely, uploading documents, and closing incidents as appropriate in IRMA, and providing backup coverage for other Teams as needed.
• Acts as DDSO liaison to outside monitoring agencies regarding incidents, including Justice Center, OIIA, MHLS, Board of Visitors, and CQC. This includes, answering questions and assisting investigators with documentation needs.
• Provides required data and information for various audit teams. This includes preparing incident documentation for litigation, audits, and surveys as requested, tracking, and trending incidents, tracking and completing Jonathan’s Law Requests, and CAP Audits.
• Monitors all entries into IRMA. Assists and ensures appropriate protections are put in place.
• Provides consultation and training to DDSO staff in Incident Management issues. This includes Training NEO, Supervisor’s Training, new Incident Management Committee Members, TTL training, etc. Triages daily calls regarding incidents and provides guidance to staff calling.
• Attends Core Leadership meetings weekly to review incidents.
• Facilitation of Mortality Review Response efforts and completion/submission timely.
• Develop and implement processes to improve efficiency in adherence with the Agency’s Quality Improvement Plan.
• Interfaces with OPWDD Central Office Incident Management Unit pertaining to Incident management issues.
• Assists in policy development related to Incident Management.
• Assists teams with submission of Corrective Actions Plans.
• Travel will be required for this position.
• Other duties as assigned.
