Below are the minimum qualifications for Provisional Appointment:
You must be a qualified employee of the New York State Office for People With Developmental Disabilities and have had one year of permanent competitive or 55-b/55-c service as a Habilitation Specialist 1, Habilitation Specialist 1 (Spanish Language), or Habilitation Specialist 1 (Manual Communications).

Duties Description POSITION DESCRIPTION/PRIMARY DUTIES AND RESPONSIBILITIES:

On behalf of the Incident Review Committee, the Habilitation Specialist 2 would function as lead Incident Review Coordinator assigned to the Incident Review Regulatory Compliance Unit (IRRC) to process incidents, provide technical assistance to Treatment Team Leaders, and other State Operated Program Managers to comply with New York State Codes, Rules and Regulations (NYCRR), Part 624 Implementation of the Protection of People With Special Needs Act, which includes Jonathan’s Law.
The unit processes 350 to 450 investigations a year. Processing an incident involves interacting with and notification to the OPWDD Incident Management Unit; completing a quality assurance check of the OPWDD 147 incident reporting form, ensuring timelines mandated by regulations are met, and communicating deficits to the filing Treatment Team; problem solving feasible protections; ensuring the completion of OPWDD form 163, Personal Representative Notification Form, interacting with Justice Center Investigators to provide initial incident reporting information; completing/overseeing data entry into the IRRC database and the Incident Report and Management Application (IRMA); sending OPWDD 147 incident reporting forms to MHLS, Board of Visitors, and Consumer Advisory Board; and overseeing clerical staff in creating incident files.
Other duties include:
• Contact and coordinate with the various Teams regarding incidents as they populate in the Incident Report and Management Application, track the progress of regulatory required documentation.
• Complete a quality assurance check of the OPWDD 147 incident reporting form, ensures timelines mandated by regulations are met, and communicate deficits to the filing Treatment Team.
• If necessary, problem solve with Treatment Team feasible initial protections.
• Ensure all needed notifications have been made, including the completion of OPWDD form 163, Personal Representative Notification Form, OPWDD 148, Report of Actions Taken.
• Track completion of investigative and Special Review Committee recommendations.
• Upload documents and complete incident closures in the Incident Report and Management Application.
• Send OPWDD 147 incident reporting forms to MHLS, Board of Visitors, and Consumer Advisory Board.
• Interact with Justice Center or OIIA Investigators to provide initial incident reporting information.
• Complete/oversee data entry into the IRRC database (Microsoft Access) and the Incident Report and Management Application (IRMA).
• Assign investigators for Agency Lead investigations.
• Using Access to create and distribute to the Teams the Monthly Investigation Status reports.
• For the QM Director, prepare a weekly report on current outstanding initial protection verification documents.
• NEO PRAISE training, mailing of 148s, and closing cases.
• Other duties as determined by the QM Team Leader
