Minimum Qualifications MINIMUM QUALIFICATIONS FOR PERMANENT APPOINTMENT:
• Reachable on the Civil Service List for Habilitation Specialist 2 OR currently permanent in a title eligible for 70.1 transfer per Civil Service Rules and Regulations.
MINIMUM QUALIFICAITON FOR PROVISIONAL APPOINTMENT:

• In the event that there is not a valid eligible list, candidates must be a qualified employee of the New York State Office for People With Developmental Disabilities and have had one year of permanent competitive or 55-b/55-c service as a Habilitation Specialist 1, Habilitation Specialist 1 (Spanish Language), or Habilitation Specialist 1 (Manual Communications).
• Only the title(s) listed is qualifying for this provisional appointment.
Note: If you are appointed provisionally, you must take the next scheduled Civil Service examination for this title and your score must be reachable on the resulting eligible list before you can be permanently appointed. If you do not pass the examination or achieve a reachable score, you may not be retained. Service as a provisional appointee is at the discretion of the appointing authority.

Duties Description • Support individuals with intellectual and developmental disabilities in developing job readiness and vocational skills
• Provide coaching and facilitate development of vocational, communication and self-advocacy skills
• Develop Staff Action Plans that address the outcomes reflected in the Life Plan as well as the needs and interests of assigned individuals
• Ensure that all documentation is entered in the electronic health record and maintained in accordance with regulation and policy
• Work cooperatively with day program staff, clinical staff, and residential teams to support achievement of person-centered goals and valued outcomes
• Provide oversight to Habilitation Specialist 1 and program activities
• Attend Life Plan meetings and collaborate with individuals, family members, advocates, care managers, and service providers to report progress and identify goals for work activities
• Document and implement appropriate safeguards to ensure the health and well-being of individuals while participating in program activities
• Monitor and document individuals’ progress in achieving their work-related goals, including monthly summary notes
• Ensure activities are person-centered and meet individuals’ needs
• Assist with planning community inclusion and meaningful activities to support person-centered valued outcomes
• Explore and identify opportunities for paid work activities; schedule and transport individuals to program activities
• Assist with documentation and investigation of injuries and incidents that occur during program hours
• Field work required
• Additional duties as assigned
