Minimum Qualifications *Permanent Transfer Candidates: current NYS employee with one year of permanent or contingent - permanent service as a Community Health Program Manager 1 (CHPM1), OR current Department of Health employee or other current NYS employee with permanent or contingent - permanent service as a G-25 and designated as eligible to transfer under Section 52.6 of the Civil Service Law OR current DOH or other NYS employee with one year of permanent or contingent-permanent service in a G-25 and eligible for transfer under Section 70.4 of the Civil Service Law by virtue of possessing the minimum qualifications below OR **Provisional Candidates: any individual meeting these minimum qualifications: A Bachelor’s degree AND 6 years professional public health experience in a governmental public health agency or public health program that receives funding from a public health governmental agency. Of the six years required experience, two years must include supervision of professional staff and the following management responsibilities: program planning and implementation, AND program evaluation and monitoring, AND development and implementation of policies and procedures.

PREFERRED QUALIFICATIONS: A Master’s degree in Public Health, Public Administration or a related field; professional work experience related to the management of a large statewide program; professional work experience in public health, epidemiology or a related field; experience with various software and databases, significant experience with program planning, implementation and evaluation, and excellent organizational and communications skills.

Duties Description Reporting to the Director of the Bureau of Immunization, the Community Health Program Manager 1 (CHPM1) will oversee the daily management of a multi-faceted federally funded Human Papillomavirus (HPV) vaccination program. Responsibilities include oversight of a variety of activities, both administrative and programmatic, and the recruitment and oversight of staff working on this program. Duties will include public health outreach to the public and providers, activities related to the management of HPV disease through immunization, and coordinating programs and services with other units in the Department, including the Public Affairs Group and the Cancer Control Program. The CHPM1 will also solicit external stakeholder input and work with local health departments to promote and increase HPV immunization rates, as well as be responsible for overall program planning, coordination and implementation, and ensure that the proposed activities are completed in a timely manner consistent with program requirements. The CHPM 1 will prepare program contracts and budgets as required by the grant and will monitor contract and grant expenditures and be responsible for all federal requirements and reports,. and serve as the liaison with the Center for Disease Control on this program.

Additional Comments *Full time, permanent appointment for those eligible for transfer. **Provisional, pending examination for all others. Travel, within and outside of NYS, to locations that may not be served by public transportation is required.
