Job Specifics

Minimum Qualifications Candidates must be in title, or eligible for 70.1 transfer, or reachable on the Civil Service Eligible List. Candidates must be currently certified by the American Health Information Management Association as a Registered Health Information Administrator (RHIA) or as a Registered Health Information Technician (RHIT). Refer to the GOT-IT website for qualifications for provisional appointment.

Duties Description POSITION DESCRIPTION:
This employee will perform technical activities involving the maintenance, review, and use of medical records within Region 2 Broome OPWDD. They will review diagnostic and therapeutic medical records for completeness and accuracy to ensure compliance with accreditation and legal requirements; ensure records reflect the current and future treatment needs of individuals receiving care; supervise and perform coding of diseases, diagnoses, operations, and procedures; participate in committees concerned with the clinical and administrative policies and procedures governing medical records when assigned; and assist and train facility staff in resolving problems with proper medical records maintenance and use as assigned. They will perform a wide variety of activities related to the receipt, review, data-entry, maintenance, storage, transfer, and disposal of records.

PRIMARY DUTIES AND RESPONSIBILITIES:
• Review records for completeness and accuracy to ensure compliance with regulatory, and legal standards and requirements.
• Requires strong computer abilities, in addition to communication and data entry skill to support the Health information management office in day to day operations.
• Determine that the necessary components are present and complete in the record; notify appropriate staff of discrepancies.
• Review admission and discharge charts to determine accuracy of documentation; directly follow up with appropriate staff when deficiencies occur to ensure records reflect the current and future treatment needs of individuals.
• Collect, review, and process all medical and clinical services documentation of facility staff, as required for Medicaid funding.
• Select and abstract information from the record; as needed.
• Correspond with individuals and entities regarding inquiries for information from records in accordance with established policies, procedures, and legal standards.
• Apply and implement legal and regulatory policies, procedures, and requirements for access and disclosure of Protected Health Information (PHI), and report privacy issues and breaches.
• Preserve, prepare and file records for archive storage in accordance with established policies and procedures.
• Provide technical assistance and guidance to staff on records maintenance and use, including records contained in automated patient information systems.
• Collaborate with staff in resolving problems with records maintenance and use.
• Prepare, retrieve, and participate in data collection and presentation for clinical and administrative review of records policy and procedures.
• Coordinate with administrative, clinical, and clerical staff to retrieve data and information.
• Train staff in records keeping procedures and data management used to meet local, State, and federal guidelines.
• Collaborate with committee participants concerned with clinical and administrative policies and procedures governing records.
• Refer records that deviate from established standards to supervisor for further investigation.
• May supervise lower level clerical and support staff.
• Other duties as assigned.
