Minimum Qualifications Minimum Qualifications:
For qualified employees of New York State: One year of satisfactory experience as an Occupational Therapist.

For all other qualified individuals, candidates must be licensed and currently registered to practice as an
Occupational Therapist in New York State AND have one year of post licensure occupational therapy
experience.

Duties Description PRIMARY DUTIES AND RESPONSIBILITIES:
The Occupational Therapist is responsible for all aspects of occupational therapy service delivery and is
accountable for the safety and effectiveness of the occupational therapy service delivery process. The
occupational therapy service delivery process involves evaluation, intervention planning, intervention
implementation, intervention review and targeting of outcomes and outcomes evaluation.
• Interviews individuals, family and caregivers to evaluate current functional level and to determine
individual rehabilitation plan for treatment.
• Selects, administers and scores written and performance evaluations and assessments to determine
present level of functioning in such areas as performing the activities of daily living, strength and
dexterity, gross and fine motor skills, interpersonal relations, communications, memory, coordination
and perceptual and problem solving skills and interprets test results.
• Develops and implements individual and group treatment plans describing the treatment strategy,
purpose and anticipated outcomes.
• Determines equipment, supplies, space and staff needed to implement treatment plans to assist
individuals to attain their therapeutic goals.
• Motivates and assists individuals in participating in therapy treatment interventions.
• Modifies individual treatment plans/goals to reflect improvements or changes and response to
treatments prescribed.
• Makes follow up treatment intervention and/or discharge recommendations to be implemented by the
individual family, caregivers and/or direct care staff.
• Determines and monitors need for adaptive equipment and other aids to improve functional skills and
abilities and may design, adapt and fabricate such aids.
• Instruction to staff and individuals on use of adaptive techniques and equipment.

• Recommendation of rehabilitation needs in the individual's work, living, educational and social/leisure
environment consistent with individual strengths, needs and learning styles.
• Instructs individuals in the development of the skills and techniques needed to participate in social and
rehabilitation activities, performing activities of daily living and learning work skills consistent with
learning style.
• Prepares written occupational therapy plans for each individual in conjunction with treatment staff from
other therapy programs.
• Supervises and conducts occupational therapy activities.
• Administers and monitors specific treatment modalities, such as sensory integration, relaxation, facilitation and neuro-developmental techniques, etc.
• Works with individuals, family, caregivers and community agencies to plan and provide continuity of
care.
• Plans for continuity of care upon discharge or movement to a less restrictive community-based setting.
• Instructs individuals, family and other caregivers regarding rehabilitation techniques and approaches.
• Recommends physical modifications in the living, work and educational environments to facilitate
social/leisure reintegration into the community.
• Monitors individual's adaptability and recommends changes to improve individual's adjustment.
• Observes, evaluates and documents individual's progress to determine response to treatment
interventions and treatment goals.
• Attends individual case reviews and team meetings to represent the occupational therapy components
of the treatment plan.
• Coordinates and integrates occupational therapy activities with those of other disciplines.
• As member of a treatment team identifies, writes or assists other disciplines in identifying and writing
goals for individuals and reviews goals in conjunction with other staff.
• Instructs other occupational therapy staff and service providers in therapy techniques and objectives.
• Supervises Occupational Therapists, Occupational Therapy Assistants, students and other staff.
• Comply with Sunmount operational policies and procedures.
• Participate as key member in the community therapeutic dining team.
• Other Duties as Assigned.
