Minimum Qualifications Must be currently in the title of Developmental Disabilities Program Specialist 2; OR
Must be reachable on the current Civil Service List; OR
One year of permanent competitive service in a title eligible for transfer under Section 70.1 or 52.6 of the NYS Civil Service Law

Duties Description As the Community Services Team Leader, duties will include, but are not limited to:

Overseeing and supervising all employees who provide services in the following roles:
Family Support Services
Medicaid Services Enrollment staff
Capacity Management Coordinator
CSS Liaison
Willowbrook Liaison
Employment/SEMP Coordinator
School Transition Coordinator
CAH/Home Care/B2H Liaison

Conduct monthly meetings with staff
Meet with the Community Service Administrator weekly to review and provide updates on all SI Regional activities
Provide monthly reports to be submitted to the Deputy Director and Director
Make presentations when needed on behalf of SI Region Office, and make trips to Albany when required
Provide technical support to voluntary agencies who have programs funded by OPWDD
Participate in all Eligibility meetings, Vacancy Management Meetings and Region 4 meetings

Additional Comments ALL OPWDD EMPLOYEES MUST BE ELIGIBLE AND MAINTAIN ELIGIBILITY FOR FULL AND UNCONDITIONAL PARTICIPATION IN THE MEDICARE AND MEDICAID PROGRAMS. CONTINUED EMPLOYMENT WILL DEPEND ON MAINTAINING ELIGIBILITY.
