Minimum Qualifications MINIMUM QUALIFICATIONS:
1. Must currently be a permanent Developmental Disabilities Program Specialist 1;
OR
2. Must be active and reachable on the Civil Service eligible list for Developmental Disabilities Program Specialist 1;
OR
3. Eligible for 70.1 transfer by way of current permanent status in a title deemed comparable to Developmental Disabilities Program Specialist 1 by the Department of Civil Service (Must also be within 2 salary grades of the targeted title).
Duties Description • Support the functions and processes related to “Self-Direction” of which CSS (Consolidated Supports and Services) is currently the means to achievement.
• Assist applicants in determining their Personal Resource Account (PRA) based on their assessed needs and establish a budget using funds from their PRA.
• Explain Self-Direction/CSS to families, agencies and local government to support and act as a liaison to them in order to support the program.
• Complete intakes for Self-Directed/CSS programs via phone or meetings with individual and his/her Circle of Support.
• Be knowledgeable of the CSS plan and budget template and review the plan’s safeguards.
• Return calls in a timely manner.
• Maintain current files on all applicants and their plans.
• Maintain Broker listings, trainings and training requirements.
• Provide Self-Direction trainings to current Brokers and potential new Brokers.
• Provide assistance to Brokers, especially new, in order to ensure plans meet Self-Direction guidance and person’s needs.
• Participate on Committee to review CSS/Self-Directed Plans.
• Monitor participant satisfaction with his/her plan and remedy areas of dissatisfaction as applicable.
• Maintain a caseload of independent individuals living in Rockland County via “MSC – like” functions and services. Assist and oversight might include housing paperwork, monthly notes, scheduling medical appointments, transportation, etc.
• Display a high-level of independent professional judgment on complex issues.
