Duties Description The Office of Mental Health (OMH) is currently recruiting to fill a Mental Health Program Specialist 2, G-23 position in the Division of Adult Services and Managed Care, Bureau of Rehabilitation Services and Care Management. The Bureau has responsibility for multiple program areas including the OMH licensed mental health program called Personalized Recovery Oriented Services (PROS); the evidenced-based practice model, Assertive Community Treatment (ACT) program; and the transition of the Targeted Case Management (TCM) programs to Health Home Care Management.
The selected candidate will work within the Care Coordination Unit on the implementation of Health Homes, the transition into Managed Care/Health and Recovery Plans (HARP), and will be responsible for providing oversight, technical assistance and guidance to counties for ACT and Care Management programs. Specific duties include, but are not limited to, the following:
• In partnership with OMH’s five field offices, serve as liaison to counties and community mental health providers and participate in ongoing policy and program planning for care coordination and other activities as assigned.
• Provide clinical and administrative expertise and technical assistance to counties and community mental health providers regarding care coordination and other activities as assigned.
• Monitor Unit programs, via on-site visits and OMH web-based reporting systems, to ensure the continued provision of appropriate and effective services.
• Support ACT and Care Coordination programs in their transition to Health Homes.
• Implement and oversee care management initiatives related to the SMI population including but not limited to Health Home Plus (HH+).
• Support the implementation of the Managed Care Behavioral Health “Carve-In” under New York State’s Medicaid Redesign.
• Work in partnership with the Department of Health (DOH) to execute the aforementioned initiatives.
