Minimum Qualifications MINIMUM QUALIFICATIONS:
1. Currently be employed by New York State as a permanent or contingent permanent Community Mental Health Nurse, OR in a title eligible for 70.1 transfer, OR
2. Reachable for appointment on the Civil Service list.
3. A bachelor’s or higher in one of the areas listed below (*) And two years of professional clinical nursing experience, including at least one year as described below (**) OR:
4. 30 College credits in the areas listed below (*) IN ADDITION TO
Associates degree in nursing or completion of a three-year nursing program AND three years of professional clinical nursing experience, including at least one year as described below (**).
(*) Acceptable areas include nursing, health care administration, public administration in a health care area, public health, community mental health, community health counseling, psychology, sociology, biology, social work or human services. The 30 credit may include up to two statistic courses.
(**) One year or experience in providing direct services to Individuals diagnosed with mental illness, mental retardation, developmental disabilities, or alcohol or substance abuse in a community setting. A master’s degree in community mental health may be substituted for this experience only.

Duties Description Work as a supervising nurse in Brooklyn Community Cluster IV and may function as a residential nurse, as need. Ensure Case management of 4-5 IRA/ICF’s who have 44-50 individuals. The CMHN will oversees and supervises RNs and LPNs who will provide supervision to the Direct Support Assistant’s in their role in providing Direct Support Assistants health care per the Nurse Act Exemption. The nurse ensures all medical follow up is done. Nurse is responsible for teaching nursing plans of service and medication administration which are vital activities to regular operations.
Supervises overall individuals’ health care in assigned homes working with Team Leader, to implement optimal care. Assess individuals on an ongoing basis, identifying needs and updating discharge transition.
Supervises the health care of individuals living in Intermediate Care Facility by working with the ICF Staff, to implement optimal care.
Working with the residence Program Manager to coordinate appointments for primary visits, consultant services and required tasks.
Provides oversight to individuals when hospitalized including the discharge transition.
Ensures that medications and treatments are administered according to approved policies and regulations. Provides ongoing oversight to staff administering medications.
Maintain Nursing coverage for respective Cluster. As well as, participate and maintain Nurse On-Call Rotation.
Provide nursing component to the Interdisciplinary Team.
Provides training and oversight to staff in the area of medication administration, OSHA (Blood Borne and TB); and individual specific training needs.
Conduct investigations as assigned by supervisor.
