Minimum Qualifications MINIMUM QUALIFICATIONS:
? Must currently be a permanent Licensed Master Social Worker 2, grade 20;
OR
? Must be active and reachable on the current Civil Service Eligible List for Licensed Master Social Worker 2;
OR
? Eligible for 70.1 transfer by way of current permanent status in a title deemed comparable to Licensed Master Social Worker 2 by the Department of Civil Service (Must also be within 2 salary grades of the targeted title).
PREFERRED QUALIFICATIONS (not mandatory):
• Extensive experience working with individuals who have challenging behaviors and psychiatric disorders in addition to Intellectual and Developmental Disabilities.
• Excellent oral and written communication and networking skills.
• Understanding of behavior support plans and data collection.
• Proficient computer and technology skills
Duties Description PRIMARY DUTIES AND RESPONSIBILITIES:
The START Coordinator is the first point of contact for clients and the community. Coordinators link service providers with individuals in need, and work to improve the functioning of existing service elements in the community. Responsibilities will include, but not be limited to the following:
• Maintain linkages and relationships with community partners.
• Ensure the coordination of support meetings and crisis plans for individuals served through START.
• Work flex, evening, and weekend hours.
• Share on-call responsibilities to ensure 24/7 response to crisis situations within two hours of call.
• Participate in recurring meetings with START leadership, clinical team, and Resource Center program.
• Develop cross systems crisis plans, intake/assessments, intervention and outcomes and any other applicable documentation of services provided.
• Maintain active caseload of approximately 30 individuals (variability may occur depending on activity level of cases and project goals).
• Understand the programmatic and cultural nuances found across different systems of care.
• Possess the vision to conceptualize the needs of individuals’ with I/DD outside of traditional institutional models of care and work holistically with individuals and systems.
Travel: Minimal travel throughout region #3 may be required.
On-Call: Shared on-call responsibilities.
