Duties Description •While working in a secure facility provide intensive treatment to developmentally disabled adults who may have been involved with the criminal justice system, will ensure the coordination and delivery of social work.
•Performs a wide variety of social services for assigned individuals.
•Independently provides written social assessments and lends written and verbal input to Treatment team meetings.
•Assesses individuals’ social needs and resources available for optimal social functioning, then develops and implements goal directed programs to meet these needs.
•Monitors individual programs for continuity, consistency and progress as determined by the Interdisciplinary Treatment Team in accordance with ICF regulations.
•Provides one on one therapy, counseling sessions and/or provides classroom instruction to small groups of individuals’ at the Regional Intensive Treatment 1.
•Prepares and conducts in-services for in assessments, programs and other counseling interventions.
•Advocates for the needs of the individuals assigned.
•Assist in referrals to appropriate level of care.
•Provides updated information to Bureau of Revenue and Reimbursement and monitors individual financial situations and acts as individual advocate in determining funding eligibility.
•Obtains informed consent from individuals/families/legal guardians or Informed Consent Committee for any rights, restriction or restrictive intervention.
•Other duties as assigned.
