Minimum Qualifications Candidates must possess a New York State License to practice as a Registered Professional Nurse and certification by the New York State Education Department in a specialty area to practice as a Nurse Practitioner. Candidates must be eligible and maintain eligibility for full and unconditional participation in the Medicaid and Medicare programs. Failure to maintain licensure, certification and/or Medicaid/Medicare eligibility will result in the termination of employment. Van Dyke ATC is a Tobacco-Free facility.

Duties Description The patients at the facility are emotionally complex with increasingly complicated medical conditions. The Nurse Practitioner provides 24-hour, across-shift communication and care consistent with licensing standards and NYS regulations. The Nurse Practitioner will review referral agency reports, obtain medical health history. Provide medical care to all patients as needed (to include prescribing appropriate medication), provide individual and group counseling and serve as mental health advisor at the multi-disciplinary treatment team meeting. Will also assist in peer reviews and assist with managed care appeals as needed. In addition, will actively participate in activities intended to improve patient care and facility operations.
