Minimum Qualifications Promotion: Candidates must have one year of permanent or contingent/permanent competitive or 55-b/55-c New York State service as a Social Work Assistant 3 or Social Work Assistant 3 (Spanish Language); OR reachable for appointment from the Civil Service List; OR eligible for a 70.1 transfer.

Open Competitive: Candidates must have a Master’s Degree in Social Work; OR a Bachelor’s Degree in Social Work and one year of post-Bachelors experience in social work; OR a Bachelor’s Degree or higher in a human service area and two years of post-Bachelors experience in social work.

Duties Description The incumbent’s job duties will include but are not limited to the following: ensure that all program and any environmental requirements related to the Family Care homes are being met and maintained; ensure that fiscal and other providers’ needs and concerns are addressed and met consistent with the needs of the individual(s) placed in the home; act as a direct link between the Family Care provider and the sponsoring agency; conduct monthly visits of 20 or less homes; develop Residential Habilitation Plans of Care; participate in semi-annual and annual reviews; participate in the auditing process of the Family Care homes and records; monitor the records at both the office and residence; advocate for the Family Care provider and the individuals.

Under the supervision of FC Coordinator, the Home Liaison ensures a safe, homelike setting in which the Family Care providers are responsible for the overall health and safety of the individuals we serve. The FC Home Liaison will also coordinate and ensure that these individuals receive scheduled services while working closely with the Medicaid Service Coordinators, the FC nurse and the day program staff. Home Liaison provides crisis intervention, conducts training of FC providers and accompanies individuals and providers to appointments as needed.

Note: A New York State driver’s license and flexibility to travel is a plus.

Additional Comments An examination of Spanish Language proficiency will be administered to the selected candidate.
