Minimum Qualifications MINIMUM QUALIFICATIONS:

Permanent Qualifications:

Must be reachable on the current eligible list for Program Operations Specialist 3.

OR

Transfer Qualifications:

Candidate must be eligible for transfer under Section 52.6 and 70.1 of the Civil Service Law.

To be eligible to:

• Transfer under Section 52.6, candidates must have at least one year of permanent, competitive service in a qualifying title allocated to a grade 23 or higher.
• Transfer under Section 70.1, candidates must have one year or more of permanent or contingent/permanent service in a title eligible for transfer under Section 70.1 of the Civil Service Law.

Note: Under Civil Service Law, an employee cannot have two (2) consecutive upward Section 70.1 or 52.6 transfers without an intervening list appointment.

Duties Description PRIMARY DUTIES AND RESPONSIBILITIES:

A critical part of our agency transformation is focused on ensuring people have person centered planning process as part of the development of the services that they receive through the Home and Community Base Settings (HCBS) Waiver. As part of HCBS Waiver Setting regulations, OPWDD is required to ensure that there is conflict-free case management. The person in this position plays a key role in helping to establish the conflict-free case-management model for people served by the OPWDD system. The work tasks associated with this role are critical to support the achievement of good case management and person centered planning processes.

The incumbent of this position will be responsible for, but not limited to, the following activities:

• Supervise staff
• Manage the transition to conflict-free case management (CFCM) including

o Developing program standards
o Assisting communication materials
o Developing guidance materials
o Providing information for other work groups involved
o Provide training related to CFCM

• Assist with responding to questions and inquiries that come in from Developmental Disabilities Regional Offices (DDROs), voluntary providers, and state agencies on the management and operation of Medicaid Service Coordination and Plan-of-Care Support Services;
• Work with a team to carry on and improve efficient, effective and consistent communication on statewide policy;
• Provide technical support, operations assistance, and guidance on service coordination to providers, Developmental Disabilities Statewide Operation Offices (DDSOOs) and other stakeholders; assist in developing and revising curriculum as it relates to Medicaid Service Coordination (MSC) required trainings;
• Research and make recommendations for national approaches and practices related to implementing person-centered planning practices and service coordination;
• Organize and help with coordination of statewide meetings and conference calls, and videoconferences, about statuses of service coordination;
• Research and answer inquiries regarding issues that arise from the DDSOOs and/or voluntary providers and other units within OPWDD;
• Represent OPWDD on interagency committees, in intra-agency work groups, and in public conferences and forums as assigned; assist with development and preparation of presentation materials;
• Support individualized choice whenever possible, within the OPWDD service environment;
• Participate in the development, documentation, and implementation of the transition of service coordination to care management;
• Create and update resource information related to service coordination such as guidance, documents, brochures, training materials, Power Points and handouts;
• Verify that all service coordination projects meet federal and state regulations, HCBS Waiver rules, and any other statewide policies or rules.
