Minimum Qualifications • Must be currently permanent in the title of Social Worker Assistant 3. OR
• Must be active and reachable on the appropriate Civil Service eligible list. OR
• Must be eligible for 70.1 transfer by way of current permanent status in a title deemed comparable by the NYS Department of Civil Service.
• Must possess
A Master’s Degree in Social Work from a regionally accredited college or university.
OR
A Bachelor’s Degree in Social Work and one year of post bachelor’s experience in Social Work.
OR
A Bachelor’s Degree or higher in a human service area and two years of post-bachelor’s experience in Social Work.
Duties Description The incumbent will function in Day Habilitation Services and will report to the Treatment Team Leader:
1. Operationally ensures the safety and welfare of persons residing in the State Operations Intermediate Care Facility (ICF) Shirtz 1 and Shirtz 2.
2. Ensures at least minimally an annual assessment of the individual’s specific needs in terms of his/her capacity to evacuate the home in a timely manner, in the event of fire emergency and ability to manage money.
3. Regularly monitors the availability of adequate clothing, personal allowance funds, records, and other monitoring responsibilities as directed by the TTL.
4. Ensures that an individual’s plan for protective oversight is reviewed by the ITT within 30 days of any change.
5. Write and manage the CFA process and update as needed and present to the ITT on a quarterly basis.
6. Develop, review, and make modifications to individual’s goal plans.
7. Train staff on goal plans and conduct observations of the implementation.
8. Conduct site visits to the Day Hab. to ensure the integration of services consistent with residential services goals.
9. Ensures that within 30 days of an individual’s admission to the ICF, the ITT will meet to review the individual’s adjustment to placement, adequacy of implemented oversight and visitation schedule, and the sufficiency of any in-house guidance and training activities.
10. Ensures the completion of the DDP-2 upon admission, and the updating as needed
11. Some traveling is required.
12. Other duties as assigned by supervisor.
