Transfer: Current or previous one year permanent service as a Community Mental Health Nurse.

70.1 Transfer: In an approved Civil Service title within two salary grades with prescribed educational requirements.

Candidates must be licensed and currently registered to practice nursing in New York State, or possess a limited permit to practice nursing in New York State. AND EITHER a bachelor's or higher degree in one of the areas listed below* AND two years of professional clinical nursing experience, including at least one year as described below** OR 30 college credits in the areas listed below* IN ADDITION to an associate's degree in nursing or completion of a three-year nursing program (attach a copy of your college transcript(s) to your OC-APP)

* acceptable areas include nursing, health care administration, public administration in a health care area, public health, community mental health, community mental health counseling, psychology, sociology, biology, social work or human services. The 30 credits may include up to two statistics courses. Courses/degrees in physical education, recreation therapy, English, history, math, anthropology, languages, etc. are NOT acceptable.

** one year of experience in providing direct services to individuals diagnosed with mental illness, mental retardation, developmental disabilities, or alcohol or substance abuse in a COMMUNITY SETTING, e.g., a halfway house, visiting nurse, or a community clinic.

Duties Description POSITION DESCRIPTION:
The Community Mental Health Nurse will identify health care needs for individuals living in family care homes throughout Cayuga, Cortland, Onondaga, & Oswego Counties. The CMHN assists the family care providers in navigating the health care system and will provide training and support to the providers as well as the individuals in the Family Care homes. The CMHN will work within the framework of a treatment team to provide nursing services, monitor health care, prescriptions and medical consults, and assist Family Care Providers with medical appointments as necessary.

PRIMARY DUTIES AND RESPONSIBILITIES:
The New York State Office for People with Developmental Disabilities (OPWDD) is seeking to fill a Community Mental Health Nurse position to provide clinical support to Family Care homes in the counties identified above. Duties and responsibilities include, but are not limited to the following:
• Identification of health care needs for individuals living in the Family Care Homes.
• Ability to complete the duties as outlined in the Family Care Manual tasks for Nursing. Monitoring of all medical aspects of the individuals on a minimum three-month basis in the home.
• Assisting Family Care Providers to navigate the health care system to aid individuals in obtaining routine check-ups with their primary care physician, and other medical services as needed. Attend specialty appointments with individuals as needed.
• Work In conjunction with Home Liaison, MSC, and Family Care provider to obtain optimal health care for individuals (diagnostic studies, age appropriate screenings for preventative health care, immunizations as appropriate), and during healthcare crisis assist and/or provide linkage of services.
• Assist individual and family care team, in conjunction with MSC, to navigate community health care resources (i.e. hospital discharge planning/accessing emergency resources, nursing home referrals, enhanced residential services, pre-referrals).
• Facilitator for promoting health care and disease prevention. Provide support to individuals in their decision making by providing explanations that would support their ability to make informed decisions.
• Act as a health care resource for individuals living in family care: finding physicians in the community, assisting consumers and the Family Care Team with finding appropriate medical day programs, providing respite medical information, end of life issues, hospice care, etc.
• Provide training and support to Family are Providers and individuals to meet the regulations of the Family Care program.
• Participate in Individualized Service Planning meetings with the individual and team.

• Ability and willingness to work with multi-handicapped, including programmatic, therapeutic, health-related and other group activities.
• Effective nursing skills with strong health assessment skills.
• Ability to respond effectively to medical emergencies.
• Possess understanding of person centered planning as well as experience and active participation in an interdisciplinary treatment team.
• Ability to travel to and from various sites in a reasonable amount of time.
• Ability to work flexible hours to meet program /individual needs.
