Salary RangeFrom $53568 to $68192 Annually
Minimum Qualifications
• Must currently be permanent in the title of Social Worker Assistant 3, OR
• Must be active and reachable on the appropriate Civil Service eligible list, OR
• Must be eligible for 70.1 transfer by way of current permanent status in a title deemed comparable by the NYS Department of Civil Service.
FOR PROVISIONAL APPOINTMENT
• Must possess a Master’s degree in Social Work from a regionally accredited college or university, OR
A Bachelor’s degree in Social Work and one year of post bachelor’s experience in Social Work, OR
A Bachelor’s degree or higher in a human service area and two years of post-bachelor’s experience in Social Work.
Duties Description PRIMARY DUTIES AND RESPONSIBILITIES INCLUDE BUT ARE NOT LIMITED TO:
• Work with families who are interested in obtaining OPWDD services
• Complete eligibility assessment authorization tool, developmental disabilities profile based on conversation with the family/individual
• Provide Service Coordinator with documentation to develop plan for appropriate services
• Write letters to agencies and families as part of the FD process
• Respond to telephone inquiries regarding FD process
• Participate in Quality Review team meetings
• Assists families in identifying needed services
• Have knowledge of OPWDD eligibility process and required documents.
• Have knowledge of OPWDD Tracking and Billing system.
• Have knowledge about services available/provided by agencies under the auspices of OPWDD.
• Addition responsibilities to be determined based on unit needs
