Minimum Qualifications • Permanently in the title of Rehabilitation Counselor 2, OR

• 70.1 transfer eligible within 2 grades as determined by Civil service.

PROMOTION QUALIFICATIONS:

Simultaneous to this posting, the civil service list for Rehabilitation Counselor 2 will be canvassed. Consideration will be contingent upon reachability or eligibility in accordance with Civil Service Law.

One year of permanent service as a Rehabilitation Counselor 1.

Duties Description PRIMARY DUTIES AND RESPONSIBILITIES:

The incumbent will work under the direction of the Regional QM/IR Director and their duties will include but not be limited to the following:

• Oversee Medical Records and TABS.
• Ensure that Statements of Deficiency are addressed and a Plan of Correction developed in a timely manner.
• Be the liaison with Counsel's Office regarding obtaining documentation required for litigations.
• Ensure that policy and procedures are followed in all areas in the department.
• Coordinate OMIG audit protocols.
• Coordinate pre-survey visits in both the Residential and Day Program.
• Other duties as directed by supervisor.
