Minimum Qualifications: Promotional; One year as a Rehabilitation Counselor 1 AND a bachelor's degree in a human service field* or NYS licensure as a Registered Nurse.

Open Competitive: Either 1: a bachelor’s degree in a human service field* AND three years post-bachelor’s qualifying experience;** Or 2: a master’s degree in a human service field* AND two years of post-bachelor’s qualifying experience;** 0r 3: a master’s degree in rehabilitation counseling AND one year of post-bachelor’s qualifying experience.**

** Qualifying Experience: To qualify for the examination you must have professional experience providing vocational, psychosocial, or rehabilitative/habilitative services in an agency primarily serving individuals diagnosed with developmental disabilities, mental illness, or addictions. Internship experience does not count if it was credited toward the qualifying degree.

Duties Description PRIMARY DUTIES AND RESPONSIBILITIES:
• Works with Team Leader and Developmental Assistant III to assure that issues related to staffing, physical plant, and budgeting are impacting positively on the active treatment process.
• Coordinates the development of individual’s rehabilitation/vocational goals and objectives with the treatment team and staff to develop an active treatment plan.
• Monitors individual programs monthly for continuity, consistency and progress as determined by the Interdisciplinary Treatment Team in accordance with ICF regulations.
• Assures that individual programs are developed in behavioral measurable terms and documentation in accordance with regulations and all active treatment mandates.
• Advocates for the needs of the individuals assigned. Monitors individuals' treatment planning, implementation, and documentation process to ensure that services are provided as mandated and that individuals receive the services that they need.
• Visits residences and program areas regularly to observe program implementation.
• Schedules annual team meetings, quarterly team meetings and special team meetings as required by individual need. Assures that schedule is flexible to assure attendance of appropriate team members, family and other DDSO’s.
• Collects and reviews assessments to identify and synthesize needs, strengths and recommendations prior to reviews. Notifies appropriate team members of discrepancies and attempts to resolve them prior to meeting.
• Chairs team meetings as QMRP.
• Ensure eligibility documentation in CFA and accompanies IUR during review.
• May accompany CMS & DOH in facilitating their reviews.
• As QMRP must have precise knowledge of individuals’ records.
• Complete monthly QMRP Notes
• Ensures timelines are met for referrals and Trips.
• Reviews medical consults monthly with RN.
• Identifies and monitors use of Adaptive Equipment
• Participates in various committees as determined by the Treatment Team Leader.
• Oversees compliance with regulations for survey.
• Supervise the Habilitation Specialist 1s
• Other duties as assigned.
