Job Specifics
Minimum Qualifications Candidates from outside or within State Service can be considered for hire under the Hiring for Emergency Limited Placement – Statewide (NY HELPS) program if they have a bachelor’s degree in a human service field and 2 year of social work experience.
Substitution: Master’s degree in social work can substitute for 1 year of specialized experience. OR
Candidates within State Service can be considered for a competitive class appointment if they are reachable on the current eligible list. OR
Candidates from within State Service can be considered transfer to a competitive class position if they have one year of permanent competitive or 55-b/55-c service in a title eligible for transfer under Section 70.1 of the Civil Service Law. OR
In accordance with Section 52.7 of the Civil Service Law, this notice advises all employees of the Office for People with Developmental Disabilities of our intention to conduct a non-competitive promotion examination for the position of Social Work Assistant 2 at Manhattan Psychiatric Center. Permanent appointment may be made by non-competitive promotion of an applicant who meets the position’s minimum qualifications if all requirements of non-competitive promotion are met.
The promotion field for the Social Work Assistant 2 position consists of candidates who have 1 year of permanent service as a Social Work Assistant 1.
Duties Description POSITION DESCRIPTION:
As the housing liaison on the region 4 capacity management team, the incumbent will manage a caseload of individuals seeking placement in a certified residential opportunity including Intermediate Care Facilities (ICF) and Individual Residential Alternatives (IRA). The housing management team is a regional team and manages referrals for individuals from all 5 boroughs.
PRIMARY DUTIES AND RESPONSIBILITIES INCLUDE BUT ARE NOT LIMITED TO:
1. Review individual’s referral clinical information so that there is an understanding of their needs. As necessary, the housing liaison will follow up with the referring party to request updated clinical information that better represents the individual’s current needs.
2. Monitor residential vacancies and make referrals to voluntary and state-operated residential programs with vacancies that match the needs of the individual
3. Interface with care managers, hospitals, and nursing homes who have referred individuals for placement
4. Using the SharePoint system, track Certified Residential Opportunities referrals and follow-up with agencies on a timely basis to ensure screenings are occurring and screening outcomes reported
5. Run Access to Residential Options Committee (AROC) meetings with providers
6. Interface the Care Manager and CSIDD (NY START) to discuss the individual’s needs as they wait for placement, ensuring that the individual is receiving the services to best support them given their current circumstances.
7. Review individual’s DDP2 scores and interface with the High Needs Funding coordinator, then advise potential providers what acuity level the individual may qualify for. As necessary, re-administer the instrument when the individual’s scores don’t reflect their current functioning/circumstances.
8. Enter information into the Residential Opportunities database and SharePoint systems
9. Assist with finding placement for individuals aging out of residential schools who are identified as emergency need and not matched with a residential provider
10. Use the TABS/CHOICES system to review other services the individual is receiving
11. Additional responsibilities to be determined based on DDRO needs

Additional Comments Please note:
• If you are currently a New York State employee serving in a permanent competitive qualifying title and eligible for a transfer via Section 70.1 of the Civil Service Law, you may be appointed to this title as a permanent competitive employee.

• The assignment of PS&T employees will be made in accordance with any agreed upon Memorandum of Understanding between PEF and the local DDSO regarding the posting of job vacancies
