Minimum Qualifications Candidates must be reachable for appointment on the current Program Operations Specialist 1 eligible lists (00233 Transition or 25565 Open Competitive) or a current New York State employee eligible to transfer under Section 52.6 or Section 70.1 of the Civil Service Law.
Duties Description OPWDD’s MFP program facilitates individuals moving from institutional residential settings into qualifying community settings with person centered planning and follow-up to ensure each person’s needs are met and their quality of life is improved. This position fulfills a critical function in the OPWDD Division of Service Delivery and will facilitate connection between the OPWDD Money Follows the Person (MFP) program and newly established NYS DOH-contracted vendors responsible for statewide outreach and transition assistance to individuals residing in nursing homes, ICF-IIDs and Developmental Centers.
• Coordinating efforts between OPWDD Front Door staff, staff planning the move out of the institution, service providers and contracted transition centers to ensure consistency in community transitions and that individual needs are being met in a timely and efficient manner.
• Helping to develop guidance and field questions for OPWDD staff and provider staff assisting with transitions.
• Supporting the OPWDD central office connection to the transition center vendors and service providers to ensure clear definition of roles and processes between the vendors and state agency staff.
• Working closely with transition centers, OPWDD Central Office, State Operations and Regional Offices to ensure accurate data collection and reporting in fulfillment of federal MFP reporting requirements.
• Assisting OPWDD staff to develop and implement effective performance measurement for the transition centers.
Additional Comments All OPWDD employees must be eligible and maintain eligibility for full and unconditional participation in the Medicare and Medicaid programs. Continued employment will depend on maintaining eligibility.
