Minimum Qualifications Possession of a Bachelor’s Degree or a license to practice as a Registered Nurse in New York State; AND, two years of experience working in any of the following, or combination of the following: a New York State Office of Mental Health, Office for People with Developmental Disabilities, or Office of Alcoholism and Substance Abuse Services licensed or supervised facility; a medical facility; a child protective services organization; or as a professional advocate or service provider in the field of rehabilitation, developmental disabilities or mental health; OR a Master’s Degree in a human services or health-related field and one (1) year of experience as described above.

Duties Description Under the direct supervision of the Director of the Surrogate Decision-Making Committee (SDMC) Program, Quality Care Facility Review Specialist 1 in the SDMC Program may perform the following duties: Receiving and processing and/or reviewing cases received from health care providers, advocates and other organizations regarding individuals served by the mental hygiene service delivery system; reviewing documents for completeness including medical necessity; individual’s eligibility and availability of a surrogate decision-maker; communicates orally and in writing with a variety of individuals, including their supervisors, agency program managers, other supervisors and employees, subordinates, staff from other state agencies and outside organizations.
Specific tasks include:
• Reviewing the present medical condition/proposed treatment in relation to the date of the onset of the problem, including relation to the date of the onset of the problem, including information about prior areas of difficulty or disability, determining the extent of the previous treatment modalities for each condition or disability identified;
• Evaluating laboratory tests and preparing records of findings of physical and developmental assessments in a systematic and accurate form;
• Participate in systemic studies and prepare reports resulting from these studies;
• Obtaining additional information from the provider, physician or dentist to ensure the case is properly prepared as required;
• Maintaining accurate records of all cases submitted to the Unit;
• Entering case information into the database as information becomes available during case preparation;
• Assisting agencies and facilities to prepare cases and explaining medical declarations;
• Participating in systematic studies and regular review of forms, policies and procedures.

Additional Comments The New York State Justice Center for the Protection of People with Special Needs (Justice Center) is charged with protecting over one million New Yorkers under the care or jurisdiction of six state agencies. The following list outlines several of the Justice Center’s key responsibilities:

• Advocate on behalf of individuals with disabilities and provide oversight of quality of care.
• Operate a centralized, statewide 24-hour hotline and incident reporting system staffed by trained personnel.
• Operate a statewide register that contains the names of individuals found responsible for egregious or repeated acts of abuse or neglect.
• Centralize and perform functions relating to criminal history background checks.
• Investigate serious allegations of abuse and neglect.
• Prosecute criminal acts of abuse and neglect.
• Analyze abuse patterns and trends in order to prevent future occurrences.
