Minimum Qualifications
• Must be currently permanent in the title of Licensed Master Social Worker 1.
OR
• Must be active and reachable on the appropriate Civil Service eligible list
OR
• Must be eligible for 70.1 transfer by way of current permanent status in a title deemed comparable by the NYS Department of Civil Service
OR
PROVISIONAL APPOINTMENT:
• Master’s Degree in Social Work from a regionally accredited college or university.
OR
• Bachelor’s degree in Social Work and one year of post-bachelor’s experience in social work.
OR
• Bachelor’s degree or higher in a human service area and two years of post-bachelor’s experience in social work.
AND
• Licensure and current registration in New York State as a Licensed Master Social Worker.
Duties Description Under the direct supervision of the Licensed Master Social Worker 2 the candidate will be responsible for (but not limited to) the following tasks:
1. Interface with hospitals who have individuals in their care
2. Review clinical evaluations to determine OPWDD eligibility
3. Write comprehensive psycho-social summaries when necessary
4. Make appropriate residential referrals based on individual’s needs
5. Interface with agencies providing residential services
6. Complete necessary forms needed for referrals – level of care form, complete EAA tool (eligibility, assessment, authorization tool)
7. Participate in monthly eligibility community of practice
8. Complete Level of Care forms
9. Additional responsibilities to be determined based on unit needs
Additional Comments • For CSEA positions: Local Bidding Agreements will be honored.

• All OPWDD employees must be eligible and maintain eligibility for full and unconditional participation in the Medicare and Medicaid programs. Continued employment will depend on maintaining eligibility.
