Duties Description POSITION DESCRIPTION:
ncumbents will manage Team to assist and support individuals in acquiring, retaining, and improving the self-help, socialization, and adaptive skills necessary to live successfully in home and/or community-based settings, in order to help individuals lead better lives; develop and implement person-centered plans for habilitation services; assist in setting goals; and monitor individuals’ progress in achieving their valued outcomes and ensuring compliance with documentation.
PRIMARY DUTIES AND RESPONSIBILITIES include, but not limited to:
• Manage a team of QIDP Reviewers that support habilitation service recording and documentation in accordance with our standards of practice.
• Ensure completion of required habilitation documentation in accordance with agency and program requirements.
• Ensure Daily Service Recording and supporting documentation is reviewed as required to determine and identify the appropriate billing code.
• Ensure participation in activities which support the program(s) and/or the unit’s approach to providing optimum care for all individuals and documentation that meets compliance.
• Ensure participation in Team Meetings and/or ad hoc meetings, as needed, and take minutes when required.
• Participate in Programmatic Self-Assessment and Regulatory Plans of Corrective Action (POCA) development, implementation and monitoring as relevant to the QIDP role.
• Complete and maintains habilitation records and required documentation in accordance with agency standards.
• Participate with other Team members in a variety of individual, Team and continuous improvement meetings and forums as scheduled to promote comprehensive and individualized services.
• Manage Placement Services for SIDDSO, overseeing placement process, communicating with outside entities (families, Care Managers, Regional and Central Offices, schools, hospitals, Team members, MHLS, etc.) in a fast-paced, multi-faceted environment
• Review and ensure timely submission of documentation
• Serve as chairperson for weekly Screening Team meetings
• Serve as chairperson for Bi-weekly Movement Meetings
• Provide weekly updates regarding current vacancies and new needs
• Maintain supervisory duties for a Team of 2-4 Habilitation Specialist 1s working in different stages within the process.
• Must have the ability to adjust work hours to meet operational/supervisory needs including but not limited to meetings, trainings, and/or off shift visits.
• Complete additional tasks, as assigned, to meet departmental needs.

Minimum Qualifications Promotion:
One year of service as a Habilitation Specialist 1 and reachable for appointment on the current Civil Service Eligible List for this title.
Transfer:
Candidates must have one year of permanent competitive or 55-b/c status in the posted title and be eligible for lateral transfer; OR be eligible to transfer via Section 70.1, 70.4, or 52.6 of Civil Service Law. The transfer cannot be a second consecutive transfer resulting in an advancement of more than two salary grades.

Additional Comments • Please note that starting salary for this title reflects the Downstate adjustment. Posted Hiring Salary includes the sum of the statutory hiring rate for PS&T Grade 17 ($63,361) and Downstate adjustment ($4,000). Downstate adjustment is also added to the job rate ($80,655).
• If you are currently a New York State employee serving in a permanent competitive qualifying title and eligible for a transfer via Section 70.1 or 52.6 of the Civil Service Law, you may be appointed to this title as a permanent competitive employee.
• The assignment of PS&T employees will be made in accordance with any agreed upon Memorandum of Understanding between PEF and the local DDSO regarding the posting of job vacancies.
• Preference will be given to OPWDD employees impacted by closures. If you are being impacted by closure, please indicate this on your resume.
• All OPWDD employees must be eligible and maintain eligibility for full and unconditional participation in the Medicare and Medicaid programs. Continued employment will depend on maintaining eligibility.
