Currently a permanent Registered Nurse 2 (Community). OR

Eligible to transfer via Section 70.1 of the Civil Service Law by having one year of permanent service in an approved title for transfer. Open to Public: Licensed and currently registered to practice nursing in New York State AND Two years of post-licensure clinical nursing experience. One year of this experience must have been providing direct services to individuals diagnosed with mental illness, developmental disabilities, addiction, or substance use disorders in a community setting.

Substitution: a bachelor’s degree in nursing may substitute for one year of general post licensure clinical nursing experience; and a master’s degree in nursing may substitute for two

Duties Description POSITION DESCRIPTION:

PRIMARY DUTIES AND RESPONSIBILITIES:

Under the supervision of the Nurse Administrator the incumbent will independently perform the following duties:

• Provide professional nursing services assigned to individuals in the Family Care Unit.
• Provide health care oversight to the individuals receiving Family Care services.
• Coordinates and follows up on each individuals’ health care in partnership with the Family Care Provider.
• Completes nursing assessments and the development and monitoring of plans of care to address the individuals’ health care need.
• Must effectively communicate and coordinate with other health care providers to ensure health care recommendations are carried out.
• Train Family Care Providers to ensure they follow any health care needs of the individuals in their care.

Duties Description
As a RN 2 you would work within the framework of treatment team providing nursing services to the individuals diagnosed with developmental disabilities, and their caretakers/families. You would have in person visits to identify factors in the social, psychological and environmental which may contribute to individuals’ disability; make recommendations for emergency or continuing care; suggest alternate treatment plans; teach caretakers/family methods of dealing with emergencies.

Posted Hiring Salary includes the sum of the statutory hiring rate ($72,032) for PS&T Grade 20, Geographic Pay Differential ($35,000), Increased Hiring Salary ($9,688) and Location Pay ($3,400). Maximum salary includes the sum of Job Rate ($91,381), Geographic Pay Differential ($35,000), Increased Hiring Salary ($9,688) and Location Pay ($3,400).

• The assignment of PS&T employees will be made in accordance with any agreed upon Memorandum of Understanding between PEF and the local DDSO regarding the posting of job vacancies
