Minimum Qualifications Transfer Qualifications: Any Current Registered Nurse 2
Minimum Qualifications: Possession of a license and current registration as a registered professional nurse in New York State; and two years of post-licensure clinical nursing experience. One year of this experience must have been providing direct services to individuals diagnosed with mental illness, developmental disabilities, addiction, or substance use disorders in a community setting.
A bachelor’s degree in nursing can substitute for one year of post-licensure clinical nursing experience; and a master’s degree in nursing can substitute for two years of post-licensure clinical nursing experience.
Duties Description PRIMARY DUTIES AND RESPONSIBILITIES:
Primary duties and responsibilities be inclusive of the Community setting and Family Care Program and will
include but not be limited to:
• Required to provide nursing assessment and monitor the health of individuals residing in all family care homes they are assigned to in St. Lawrence, Essex, Clinton, Jefferson, and Franklin Counties. On occasion, on a short-term basis, may need to cover for other nurses in the Sunmount area.
• May be expected to travel to other Sunmount DDSO offices for training purposes and/or for consultation/meetings with other team members.
• Will provide consultation and supervision of family care providers in their performance of health-related procedures.
• Will collaborate with physicians and other health care providers to advocate for the health care needs of individuals as needed.
• Review of medical documentation related to the individuals’ medical needs and follow up on any medical issues needing addressing.
• Ensure medication administration requirements are met.
• Will visit all assigned family care homes on no less than a quarterly basis and works cooperatively with clinical team members, individuals, and family care providers.
• Will attend clinical team meetings and meet the standards as specified in the Family Care Manual, Sunmount policy and procedures - nursing /health policy.
• Provide training to Family Care Providers and Family Care Substitute providers to meet the requirements to become certified, re-certified and maintain certification.
• Adjust schedule as needed to coordinate coverage with other RNs within the team, to provide oversight and medication recertification for AMAP staff on all shifts, to attend medical appointments, and to visit consumers in hospitals when necessary.
• Responsible for ensuring accurate and efficient medication systems within the home, including, but not limited to inventory, storage, documentation, and follow-up.
• Provide clinical supervision to LPNs in all nursing functions/direct care staff for nursing tasks as necessary.
• Ensure timely and appropriate follow-up on consumer health, medical, and psychiatric issues.
• Responsible for development of appropriate self-medication administration goals for individuals.
• Responsible for overseeing and maintaining effective infection control practices at the IRA and for monitoring immunization status of staff and individuals.
• Responsible for assessing and ensuring delivery of all aspects of medical/psychological care if needed and oversight of LPN’s/direct care staff assigned to provide the care.
• Responsible for ongoing assessment of health status, and provision of advocacy for necessary interventions along with appropriate documentation, in a timely manner.
• Responsible for timely completion of all nursing assessments and related paperwork, as outlined in the HCBS Waiver regulations.
• Responsible for ensuring compliance with 633 regulations, nursing policies and procedures, and appropriate and timely documentation.
• Responsible for development and oversight of Nursing Plans of Service as assigned.
• Responsible to train staff on Nursing Plans of Service on an ongoing basis as needed.
• Responsible for completion of non-reportable logs, medication error logs, etc., monthly for the IRA monthly report.
• Responsible for training staff in current nursing policy and procedures, individualized specific medical issues and treatments, medication administration (and recertification), and ensuring compliance through oversight and record reviews.
• Responsible for ensuring implementation of site-specific protocols for emergencies, and provision of emerging evaluations and assistance.
• Responsible for pouring and passing medications and doing treatments when needed.
• Must be able to work well with involved family members/advocates.
• Will perform various program related duties such as investigations, CPR/FA training, PPD administration, assessment of referrals and family care provider applicants, and additional duties as assigned.
• Willing to become trained as a CPR/AED/First Aid instructor if not already certified to do so.
• Other duties as assigned by RN Supervisor 1.

• Family Care caseload in St. Lawrence, Essex, Jefferson, Clinton, and Franklin Counties.

NY HELPS This title is part of the New York Hiring for Emergency Limited Placement Statewide Program (NY HELPS). HELPS Program titles may be filled via a non-competitive appointment. This means that you do not need to take an exam to qualify, but you do need to meet the minimum qualifications of the title.
If you are hired under the HELPS Program, you will be first be appointed as a temporary employee. If you are performing successfully in the position, your appointment will be changed from temporary to permanent non-competitive. Once you are in permanent status, your official probationary period will begin and you may take any promotion exam for which you are qualified. At a future date (within one year of permanent appointment), employees hired under NY HELPS are expected to have their permanent non-competitive employment status converted to permanent competitive status. You will not have to take an exam to gain permanent competitive status.
