Job Specifics

Duties Description • Have working knowledge of OPWDD state and federal regulations including Part 624, Part 633, Family Care Manual, DQI survey protocols as well as statewide and local policy and procedures.
• Conduct on site reviews of IRA, Day Habilitation, and Family Care programs to monitor compliance with regulations, determine if individuals’ support needs are being appropriately addressed, evaluate person-centered service plans and progress toward goals, and ensure that plans are coordinated, reviewed and modified as needed.
• Conduct morning and evening observations at residential, day program and other environmental settings, interview service recipients , and conduct reviews of program records, physical plant, fire drill/evacuations, and personal allowance spending to evaluate whether services are appropriate to support needs.
• Review records to assure compliance with requirements and standards.
• Document findings and recommendations to ensure treatment team members receive accurate and timely information regarding individuals’ needs, areas for improvement, and required corrections.
• Complete incident reporting, assist with incident management including tracking/monitoring of events, complete investigative reports.
• Assist the QI department in gathering statistics and information for management program reports.
• Provide technical assistance or training as necessary on topics related to regulatory compliance and quality improvement.
• As needed, provide coverage and coordinate person-centered service plans to ensure services meet individual needs; monitor and document progress on monthly notes, participate in treatment team meetings, and update/modify individual plans as indicated.
• Utilize Excel, EHR, Choices, TABS, IRMA, Acumatica, Fire Portal to organize and facilitate QI department activities.

• Proficiency in Microsoft Office (Word, Excel, PowerPoint) and Access.
• Analytical and administrative skill sets.
• Presentation and training experience.
• Familiarity with HCBS waiver service programs and Waiver service documentation.
• Familiarity with OPWDD Administrative Memorandums (ADM’s) and regulations.
• Ability to work well independently and in a team environment.

Minimum Qualifications • Reachable on the Civil Service List for Rehabilitation Counselor 2 OR
• Currently serving as a permanent Rehabilitation Counselor 2 and eligible for transfer; OR
• One year of permanent competitive service in a title eligible for transfer under Section 70.1 of the NYS Civil Service Law.

OPEN COMPETITVE QUALIFICATIONS: Bachelor’s degree in a human service field and three years of professional experience providing vocational, psychosocial, or rehabilitative/habilitative services in an agency primarily serving individuals diagnosed with developmental disabilities, mental illness, or addictions.

Additional Comments Please note: This title is part of the New York Hiring for Emergency Limited Placement Statewide Program (NY HELPS). HELPS Program titles may be filled via a non-competitive appointment. This means that you do not need to take an exam to qualify, but you do need to meet the minimum qualifications of the title.

At a future date (within one year of permanent appointment), employees hired under NY HELPS are expected to have their permanent non-competitive employment status converted to permanent competitive status. You will not have to take an exam to gain permanent competitive status.
