Duties Description Under the direction of a registered nurse, physician, or other licensed health care provider, supervises Licensed Practical Nurses and unlicensed direct staff.

• Schedules and assigns basic care and observational tasks to staff consistent with individual treatment plans to effectively support nursing care and treatment.

• Provides staff with guidelines, instructions, and standards for care; participates in staff development activities to maintain and enhance skills; and trains staff in basic individual care activities.

• Observes staff in carrying out assignments, including medication administration and management, to ensure that standards of care, guidelines, and instructions are followed; reviews records and notes completed by staff to ensure compliance with applicable requirements; and discusses finding with higher-level clinical staff.

• Meets with staff and provides feedback and suggestions for improvement in completing assigned tasks; addresses work-related questions, concerns, and issues expressed by staff; and reports to higher-level staff when necessary.

• Completes performance evaluations; tracks time and attendance; and coordinates schedule and assignment changes.

• Observes living and treatment areas; and may perform and/or monitor housekeeping tasks to ensure a safe, clean, and therapeutic environment for individuals.

• Monitors supply inventories and completes requisition forms to ensure the availability of supplies and equipment for daily activities.

Performs the tasks and activities of a Licensed Practical Nurse and provides nursing care consistent with individual treatment plans and scope of practice.

• Observes, monitors, and records individuals’ symptoms and conditions to obtain and provide information for diagnosis and treatment; identifies responses to medication or changes in health status; and reports such to a registered nurse, physician, or other licensed health care provider.

• Administers medications and nursing procedures to individuals as prescribed.

• Maintains charts and other records of individuals’ conditions and/or administered treatments.

• Attends to individuals’ personal needs, including activities of daily living and hygiene tasks, such as bathing, dressing, feeding, and grooming.

• Educates and counsels individuals and/or families regarding their health, medications, treatment goals and procedures, and behavioral interactions as appropriate.

Minimum Qualifications Promotion: possession of a license and current registration as a Licensed Practical Nurse in New York State; and one year of service as a Licensed Practical Nurse and reachable for appointment on current Civil Service Eligible List for this title.

Open Competitive: A license and current registration to practice as a practical nurse in New York State and one year of experience providing nursing care and treatment services to individuals under the supervision of a registered nurse; assisting with the implementation of treatment plans; administering, observing, and recording medication management; educating and counseling individuals and families regarding illnesses, medications, tests, and treatment goals and procedures; monitoring and recording individuals progress; and supervising direct care staff.

TRANSFER:
Candidates must have one year of permanent competitive, non-competitive or 55-b/c status in the posted title and be eligible for lateral transfer; OR be eligible to transfer via Section 70.1, 70.4, or 52.6 of Civil Service Law. The transfer cannot be a second consecutive transfer resulting in an advancement of more than two salary grades.

Additional Comments This title is part of the New York Hiring for Emergency Limited Placement Statewide Program (NY HELPS). HELPS Program titles may be filled via a non-competitive appointment. This means that you do not need to take an exam to qualify, but you do need to meet the minimum qualifications of the title.

At a future date (within one year of permanent appointment), employees hired under NY HELPS are expected to have their permanent non-competitive employment status converted to permanent competitive status. You will not have to take an exam to gain permanent competitive status.

• Please note that starting salary for this title reflects the increased hiring salary and geographic pay differential. Posted Hiring Salary includes the sum of the statutory hiring rate for CSEA Grade 11 ($47,695), Increased Hiring Salary ($6,144), Geographic Differential ($4,000). Geographic Differential is added to the Job Rate ($58,447).

• If you are currently a New York State employee serving in a permanent competitive qualifying title and eligible for a transfer via Section 70.1 of the Civil Service Law, you may be appointed to this title as a permanent competitive employee.

• For CSEA positions: Local Bidding Agreements will be honored.

• All OPWDD employees must be eligible and maintain eligibility for full and unconditional participation in the Medicare and Medicaid programs. Continued employment will depend on maintaining eligibility.
