Minimum Qualifications MINIMUM QUALIFICATIONS:

• Eligible for 70.1 transfer by way of current permanent status in a title deemed comparable to

candidate, when the exam list is produced by civil service, in order to obtain permanent status and
maintain your position.
Below are the minimum qualifications for Provisional Appointment:
You must be a qualified employee of the New York State Office for People With Developmental Disabilities
and have had one year of permanent competitive or 55-b/55-c service as a Habilitation Specialist 1,
Habilitation Specialist 1 (Spanish Language), or Habilitation Specialist 1 (Manual Communications).
Qualified Intellectual Developmental Professional (QIDP) is preferred. QIDP candidates must
possess bachelor’s degree in human services and have one (1) years’ experience working with
people with developmental disabilities.

Duties Description DUTIES:
• Will oversee, coordinate, and assist in the development and provision of Day hab services for individuals attending day hab.
• Will oversee and assist Habilitation Specialist 1’s with the development and modification of Staff Action Plans, Safeguard Summary Plans, as needed.
• Will participate in joint Residential and Day Habilitation Meetings and may be asked to chair Committees for special programming and events.
• Will coordinate with Clinical staff to ensure all individual needs are met and programs are developed/modified as appropriate.
• Will provide training and hands on assistance to Habilitation Specialist 1’s and staff in the proper delivery of daily services.
• Will provide training and oversight in the proper documentation of Medicaid Billing documents, Habilitation Monthly Notes and General notes in relation to the implementation of Day hab services.
• Will develop and maintain a good working relationship with Care Managers and Residential staff that serve individuals.
• Will attend Life Plan reviews for individuals as needed, work collaboratively with the Care Managers, and assist in the development and updating of Staff Action Plans and Safeguard Summary Plans that support the Life Plan.
• Will communicate with, develop, and maintain as good working relationship with staff to ensure the implementation of Staff Action Plans and staff resources are available for the needs of the individuals.
• Will supervise Habilitation Specialist 1’s.
• Will have oversight and be responsible for the content and organization of the individual program charts,
filing system and EHR in relation to Habilitation services.
• Will assist Habilitation Specialist 1’s to provide oversight for the direct care staff in the development and
implementation of Staff Action Plans and corresponding documentation.
• Will complete weekly and monthly chart audits to ensure documentation requirements are correct and
present.
• Will communicate with Regional Office, CCO’s and Care Managers to ensure all current changes are
brought to the Habilitation Specialist for implementation.
• Will attend and participate in all mandatory trainings.
• Other duties as assigned.

• Ability to manage time to meet the needs of the individuals, as well as documentation requirements.
• Previous documented and verifiable experience.
• Satisfactory employment history as evidenced by a review of personal history folder and supervisory
evaluations.
