3. Eligible for 70.1 Transfer by way of current permanent status in a title deemed comparable to Habilitation Specialist 2 by the Department of Civil Service (Must also be within 2 salary grades of the targeted title). OR

Duties Description OPWDD is the safety net provider for individuals in New York State with Intellectual and Developmental Disabilities. CATH homes (Comprehensive Adult Transition Homes) are being created to help facilitate the transition of young adults from residential schools both within NY State and outside of NY into our adult residential opportunities. Many of these individuals have substantial behavioral risks and poor safety awareness in the community. The CATH homes are designed to provide short term (<18 months) intensive supports to facilitate the transition to homes with less staff intense needs.


PRIMARY DUTIES AND RESPONSIBILITIES:


• Under the direction of the Treatment Team Leader, the incumbent will be responsible for two (2) CATH (Comprehensive Adult Transition Homes). The Team serves approximately 9 high intensity needs individuals. The Habilitation Specialist 2 is critical to maintain oversight for all health and safety requirements for these individuals. It is anticipated, based on the referrals received that these individuals will be involved in numerous incidents and significant behavioral and potentially medical involvement. Due to the high intensity needs and the follow up required by the Habilitation Specialist 2 for individuals with these needs, a separate Habilitation Specialist 2 is being created to ensure the timely submission of all documentation and treatment.
• Additionally, there are significant preliminary care planning needs as well as monthly planning needs/meetings for the individuals to transition out of the CATH homes which begins right at the time of admission.
• The incumbent will play a role in ensuring the referral process is complete and moving forward in a planful manner. The CATH homes will have a dedicated team for the Habilitation Specialist 2 to oversee, which will also require additional training and support for the team. The Habilitation Specialist 2 will be required to maintain the person-centered perspective to ensure the safety of all the staff and individuals at the home. Community programming may be difficult for these individuals, which means more time in the home, potentially, and greater oversight by the Habilitation Specialist to ensure community activities are being provided as per HCBS standards.

• The incumbent will provide leadership and oversight to Core Team staff to ensure that all staff understand the mission of the agency, philosophy and goals, plus their own role in the organization; maximize each individual's valued outcome and maintain a safe, sanitary environment while controlling personal service costs; ensuring appropriate training for all staff; ensuring appropriate and timely follow-up on problem issues identified by auditors and/or other individuals within and external to the agency; to participate fully and constructively as a member of the DDSOO and local administrative teams in planning, coordinating and developing a sound residential and habilitative program.

• The incumbent will provide regular training, oversight and supervision to direct care staff, DA2s, and DA1s assigned to the homes in order to clarify and ensure proper and effective service delivery of HCBS plans and Residential Habilitation plans as well as proper completion and submission of the billing forms which support our programs. The Habilitation Specialist 2 will meet with the team for Life Plan reviews as well as for monthly meetings and develop person-centered Residential Habilitation plans.

• The incumbent will develop Residential Habilitation Plans that address the outcomes as reflected in the Life Plans as well as the needs and interests of assigned individuals within the HCBS protocol. This person will need to provide supervision, directives, training and oversight to Direct Care staff in all areas of Service Delivery, document completion, techniques for effective interaction and teaching related skills to individuals. The incumbent must ensure all documents are stored securely, organized effectively, filed according to policy and protocol and neatly maintained per regulation and policy. They will also be responsible for responding to requests for document audits.

• In addition, the Habilitation Specialist 2 is responsible for:
• Regular site visits to homes.
• Ensuring records are Medicaid compliant.
• Participating in audits.
• Completing all required reports/projects as needed.
• Serve as a liaison between care managers, families/advocates.
• Other duties to be assigned by TTL.

• Although the district has other Habilitation Specialists, the role and function of the CATH (Comprehensive Adult Transition Home) Habilitation Specialist 2 is unique to this assignment. CATH homes have specific criteria which must be met, including immediate discharge planning, skills training development, community activities, and increased therapeutic contact with clinical staff. A dedicated Habilitation Specialist 2 is necessary to oversee and ensure that all components of the individuals’ habilitation needs for the CATH homes are being accomplished. There may be increased documentation and reports that the Habilitation Specialist 2 may be responsible for related to the flow of individuals into and out of these CATH homes since individuals will be transitioning into the community and other homes within 18 months of admission.
