Minimum Qualifications Non-competitive: Applicants must possess a license and current registration as a registered professional nurse in New York State; and two years of post-licensure clinical nursing experience. One year of this experience must have been providing direct services to individuals diagnosed with mental illness, developmental disabilities, addiction, or substance use disorders in a community setting. Substitution:
A bachelor's degree in nursing may substitute for one year of general post-licensure clinical nursing experience; and a master's degree in nursing may substitute for two years of general post-licensure clinical nursing experience.

Duties Description • This position will be within the Division of Statewide Services and report to the Preadmission Screening and Resident Review (PASRR) Supervisor within Central Office as part of the Bureau of Nursing and Health Services. As a PASSR Coordinator the Registered Nurse 2-Community will report to the PASRR Supervisor. In their role, the PASSR Coordinator will support the work within the bureau and respond to the direction of Statewide Services' administrators. The PASRR process is a federal requirement under the 1987 Nursing Home Reform Act 999 stemming from the U.S. Supreme Court Olmstead v. L.C. decision. The emphasis of PASRR, and the federal regulations that require OPWDD to administer assessments under PASSR, is to de-institutionalize individuals with intellectual and developmental disabilities (I/DD) and prevent inappropriate nursing facility admissions. The PASRR process embodies OPWDD’s core mission to create a system of supports that honors the individual's right to be responsible for their own life and make recommendations that supports the least restrictive settings placement recommendations for individuals with I/DD.
• In accordance with federal regulation, hospitals conduct Level I screens on all persons seeking admission to nursing facilities. If the nursing facility applicant is known or suspected to have an I/DD, the hospital must ensure that a referral is made to OPWDD for complete a PASRR Level II assessment. The OPWDD PASRR Coordinator reviews all information provided to determine if the type of care needed can or cannot be provided within the spectrum of OPWDD care. If nursing facility placement is determined appropriate, PASRR coordinators determine if specialized services, services beyond those provided by the nursing facility to support a person with I/DD, are indicated for the individual. The PASRR coordinator assures individuals have the supports needed to potentially return from the nursing facility to a lesser level of care and live as independently as possible in the community. To accomplish this, it is essential the PASRR Coordinator have strong clinical strengths, aptitudes and understanding of I/DD diagnoses, strong advocacy skills, and ability to complete timely and accurate assessments to promote an individual’s community reintegration.
• The PASRR Coordinator must seek to minimize reliance on nursing facilities for people with I/DD by being informed of alternative community-based options, effectively monitor the nursing facility population, and address requests for alternative placements in a timely manner. The tasks and responsibilities of the PASRR coordinator include, but are not limited to:
• - Work within databases and systems including TABS to review OPWDD eligibility determination for any individual referred for PASRR Level II assessment.
• - Guide referral sources to the Front Door process and OPWDD eligibility determination process (if eligibility is not determined) and advise of the any necessary assessments (e.g., comprehensive psychological, psycho-social, physical evaluations).
• - Collaborate with Front Door staff to assure that eligibility review continues while PASRR assessment occurs.
• - Communicate with hospital/and or nursing facility regarding referral package requirements and specific nursing facility identified.
• - Complete the Level II PASSR assessments, determine if specialized services are needed and should be provided by a qualified I/DD professional.
• - Determine, based upon review of the Level I screen, and materials received from the referral entity, the individual's category of determination (e.g., exempt or meets a categorical determination).
• - Assure communication with the OPWDD Regional Field Office staff about the status of admission and discharge of individuals in nursing facilities.
• - Provide in-person or virtual assessments as assigned.
• - Completion of other duties as assigned.

Additional Comments This title is part of the New York Hiring for Emergency Limited Placement Statewide Program (NY HELPS). HELPS Program titles may be filled via a non-competitive appointment. This means that you do not need to take an exam to qualify, but you do need to meet the minimum qualifications of the title.

At a future date (within one year of permanent appointment), employees hired under NY HELPS are expected to have their permanent non-competitive employment status converted to permanent competitive status. You will not have to take an exam to gain permanent competitive status.

Posted Hiring Salary includes the sum of the statutory hiring rate ($72,032) for PS&T Grade 20, Geographic Pay Differential ($35,000), Increased Hiring Salary ($9,688) and Downstate Adjustment ($3,400). Maximum salary includes the sum of Job Rate ($91,381), Geographic Pay Differential ($35,000), and Downstate Adjustment ($3,400).

• If you are currently a New York State employee serving in a permanent competitive qualifying title and eligible for a transfer via Section 70.1 of the Civil Service Law, you may be appointed to this title as a permanent competitive employee.

• The assignment of PS&T employees will be made in accordance with any agreed upon Memorandum of Understanding between PEF and the local DDSO regarding the posting of job vacancies.
