Duties Description • Works as a member of the interdisciplinary residential team.
• Schedule and attend annual and semi-annual Life Plan meetings as needed.
• Assist individuals in creation, implementation, and revisions of Residential Habilitation plans based on the individual’s needs.
• Develop new Residential Habilitation/Staff Action Plans based on individual’s valued outcomes.
• Coordinate special meetings with outside agencies/advocates to review the individual’s plan as needed.
• Ensure completion of monthly progress notes and compliance with TABS billing as required by agency.
• Develop/review/revise Safeguard Summary Plans as needed.
• Review weekly discrepancy report and follow up on staff compliance as needed.
• Conduct occasional on-site reviews to monitor compliance with OPWDD regulations.
• Maintain Residential Habilitation records in assigned IRAs. Caseload may change based on operational need.
• Provide ongoing training to IRA staff to ensure Valued Outcomes/Goals, Safeguard Summaries and required billing documentation is completed correctly.
• Review monthly data sheets/progress notes ensuring proper format is followed and is complete.
• Communicate with Day/Work programs to ensure coordination of services, ensuring incident reporting, oversight of training, assist with investigative procedures.
• Prepare all Residential Habilitation/Staff Action plans and Data sheets for audits.
• Complete investigation of incidents, gather information for the incidents.
• Establish working relationships with Care Managers and clinicians to ensure Life Plan meetings are conducted as required.
• Schedule Life Plan and special meetings.
• Ensure Residential Habilitation documents are inputted/uploaded into EHR.
• Assist with training staff to document staff actions/services into EHR.
• Additional responsibilities as assigned by supervisor.
• Travel required.
• Review EHR weekly.

Minimum Qualifications 1. Must currently be a permanent Habilitation Specialist 2, grade 17:
OR
2. Must be active and reachable on the Civil Service Eligible List for Habilitation Specialist 2:
OR
3. Eligible for 70.1 Transfer by way of current permanent status in a title deemed comparable to Habilitation Specialist 2 by the Department of Civil Service (Must also be within 2 salary grades of the targeted title). OR
4. Qualified for a provisional* appointment to Habilitation Specialist 2. (Minimum Qualifications are one-
year permanent service as a Habilitation Specialist 1). Applications will only be considered if we have insufficient applicants in the above categories.

NOTE: Provisional appointments will only be considered if there are insufficient In-title and/or Civil Service List candidates.

Valid unrestricted license to drive in New York State.
