Duties Description • Direct a Day Habilitation Program through the delivery of comprehensive day habilitation services provided by multi-disciplinary and direct care staff members at the program site and/or off-site locations.
• Maintain day program certification according to regulatory requirements.
• Supervise Habilitation Specialists, Direct Support Professionals, and clinical staff in the development of skills including pre-vocational, academic, communication, socialization and self-help activities.
• Supervise Habilitation Specialists and clinical staff in the development of individual Day Habilitation Staff Action Plans, establishment of goals, and monitoring individuals’ progress toward the achievement of those goals.
• Ensure the implementation of individual service plans by Habilitation Specialists and direct-care staff.
• Provide specialized training, when required, and guidance to staff on a day-to-day basis. Ensure required training of staff is completed as per agency requirements and individuals’ needs.
• Ensure the planning of community inclusion activities by Habilitation Specialists for each individual and the delivery and documentation of community inclusion services in accord with each individual's valued outcomes.
• Ensure that incidents are reported and documented appropriately, and recommendations for follow up actions are completed in accordance with investigator and/or IRC recommendations.
• Complete investigations as assigned for minor notable incidents occurring at LIDDSO service locations.
• Attend Life Plan meetings and work closely with the residential team, care managers, families and advocates to ensure coordination of services.
• Conduct observations and visits to off-site locations in the community to monitor and assist with program planning and service delivery at those locations.
• Applicant must obtain PROMOTE certification if not currently certified.
• Applicant must be First Aid and CPR certified.
• Other duties assigned by supervisor.

Minimum Qualifications PROMOTION:
One year of permanent service as a Rehabilitation Counselor 1 AND a qualifying bachelor's or higher degree*.

OPEN COMPETITIVE QUALIFICATIONS:
A bachelor’s degree in a human service field and three years of professional experience providing vocational, psychosocial, or rehabilitative/habilitative services in an agency primarily serving individuals diagnosed with developmental disabilities, mental illness, or addictions.

Substitution:
A master's degree in a related field can substitute for one year of specialized experience.

Additional Comments This title is part of the New York Hiring for Emergency Limited Placement Statewide Program (NY HELPS). HELPS Program titles may be filled via a non-competitive appointment. This means that you do not need to take an exam to qualify, but you do need to meet the minimum qualifications of the title.

At a future date (within one year of permanent appointment), employees hired under NY HELPS are expected to have their permanent non-competitive employment status converted to permanent competitive status. You will not have to take an exam to gain permanent competitive status.

• Please note that starting salary for this title reflects the Downstate adjustment. Posted Hiring Salary includes the sum of the statutory hiring rate for Grade 19 ($68,523) and Downstate adjustment ($3,400). Downstate adjustment is added to the job rate ($87,032).

• If you are currently a New York State employee serving in a permanent competitive qualifying title and eligible for a transfer via Section 70.1 of the Civil Service Law, you may be appointed to this title as a permanent competitive employee.

• The assignment of PS&T employees will be made in accordance with any agreed upon Memorandum of Understanding between PEF and the local DDSO regarding the posting of job vacancies.

• Preference will be given to OPWDD employees impacted by closures. If you are being impacted by closure, please indicate this on your resume.

• All OPWDD employees must be eligible and maintain eligibility for full and unconditional participation in the Medicare and Medicaid programs. Continued employment will depend on maintaining eligibility.
