Minimum Qualifications Reachable for appointment from the Civil Service eligible list; OR
One year of permanent competitive or 55b/c service as a Licensed Master Social Worker 2 or eligible for a 70.1 transfer.
Duties Description An administrative position that acts as supporting clinician to the Division of Juvenile Justice & Opportunities for Youth’s (DJJOY) Bridges to Health (B2H) Waiver Program. Candidate will handle complex issues and exercise independent professional judgment in performing those duties. Advise both clinical and non-clinical staff of clinical requirements for B2H. Review/approve mental health assessments.
Complete B2H documentation, including: Referrals, Enrollments, Discontinuances, Denials, Closures, etc. Review/approve youth’s Individualized Health Plans for the Health Care Integration Agencies (HCIAs) who create these plans.
Advise B2H providers on best practices and strategies for serving DJJOY youth. Advocate for DJJOY youth with all providers. Provide feedback to HCIAs and their sub-contractors on their service provision.
Provide technical assistance on B2H Policy and Procedure to: OCFS Aftercare, Facilities, Voluntary Agencies, Local Departments of Social Services (LDSS), Administration for Children’s Services (ACS) and other Bureaus within OCFS. Communicate regularly, clearly and professionally via Outlook and phone.
Maintain DJJOY B2H case files and databases. Prepare for, and participate in, monthly statewide conference calls/case reviews. Create reports and spreadsheets from the B2H ACCESS database.
Assist in the development of training curricula. Co-lead B2H Trainings statewide.
Provide support to Crisis Coordinator when DJJOY youth require psychiatric hospitalization. Review documentation and arrange for discharge from psychiatric hospitals.
Review mental health records for release as part of record requests.
Additional Comments Candidate should be comfortable learning and utilizing databases/computer programs for B2H, i.e., Welfare Management System (WMS), Connections (CONX), Juvenile Justice Information System (JJIS), ACCESS (B2H database of record), etc.
