Duties Description The person in this position will serve as the Community Transitions Unit (CTU) Supervisor for the Division of Service Access, Program Implementation and Stakeholder Support (SPS). This position is responsible for policy and procedure development, management, implementation and ongoing technical assistance to the field regarding helping people with developmental disabilities transition from institutional settings to community-based settings. This position will report to the Director of the Community Transitions Unit (CTU).

Duties include but are not limited to:
· Oversee the school transition process to ensure students graduating from NYS schools each year have information and access to the OPWDD adult services they need.
· Oversee efforts to develop adult services and housing for an average of 300 individuals annually who have graduated from but remain at out of state and in-state residential schools.
· Supervise Community Transition Unit staff and provide technical assistance to the Regional Office Transition coordinators.
· Oversee contact with the 50 in-state and out-of-state residential schools regarding upcoming graduates; conduct regular quarterly meetings to review case planning with a subset of 18 of the largest schools.
· Develop policies that promote a person-centered approach for the planning and service development of individuals who are transitioning from their educational programs to OPWDD adult services.
· Responsible for policy development and consistency of implementation for transition from educational settings statewide; work with Regional Office staff across the state to promote best practices for assisting graduating and graduated students and their families in securing needed residential opportunities.
· Serve as primary CTU interface with OPWDD fiscal department regarding funding for students remaining post-graduation at residential schools.

· Provide technical assistance to OPWDD staff state-wide and external entities including residential schools and adult service providers including developing trainings.

· Manage and oversee the database that monitors and tracks planning efforts for these individuals; ensure proper recording by Regional Office staff; draft reports for OPWDD Leadership and external stakeholders.

· Oversee issuance of requests for proposals to agencies across the state to plan and develop adult housing opportunities and service residential school students upon graduation.

Minimum Qualifications Promotion:
Candidates must be reachable for appointment on the current Civil Service Eligible List.

Transfer:
Candidates must have one year of permanent competitive or 55-b/c status in the posted title and be eligible for lateral transfer; OR be eligible to transfer via Section 70.1, 70.4, or 52.6 of Civil Service Law. The transfer cannot be a second consecutive transfer resulting in an advancement of more than two salary grades.
