Duties Description The Habilitation Specialist 2 will provide services to two (2) day programs for Area 3, Team 2. They will also provide coverage for Day Hab vacancies or extended leaves. Will also provide direct supervision and oversight to a minimum of two HS1 Program Habilitation Specialists to ensure services are provided for all individuals based on their needs and valued outcomes.

Duties include, but are not limited to:
• Provide supervision and guidance to the HS1s assigned to the program center
• Write, oversee implementation, and ensure data collection for Safeguard Summary Plans and Staff Action Plans
• Train all program staff in implementation of SSP’s and SAP’s, data collection, and day habilitation notes to substantiate Day-Hab billing
• Oversees individual data and maintains all the files in the program which includes the Welcome, Res Hab and Active Treatment books, and uploading files into the EHR
• Responsible to ensure Life Plans are completed with the proper supervision levels, prescribed diet plans, goals, health information, and other safeguards to ensure the safety of the individuals
• Responsible to ensure that all plans for the individual contain identical information (i.e. supervision levels, diagnoses, etc…) to ensure safety and safeguards are maintained
• Rectify Day-Hab billing issues with RSFO
• Provide and/or ensure training of all program staff as needed in response to DQI audits, regulations, and their roles in Medicaid Compliance.
• Follow up with managers in training and supervising direct care staff in the areas of individual care to ensure standards of care
• Participate in DQI and Medicaid Compliance audits, implement and oversee implementation of corrective actions as needed under the oversight of TTL
• Provide opportunities for socialization to specialized populations and high behavior needs individuals who cannot be served by a voluntary agency.
• Responsible for the health and safety of the individuals participating in the day habilitation program.
• Ensures the direct support staff complete the appropriate documentation for the individuals they are directly serving.
• Other duties as requested by the supervisor.

Minimum Qualifications Promotion:
One year of service as a Habilitation Specialist 1 and reachable for appointment on the current Civil Service Eligible List for this title.

Transfer:
Candidates must have one year of permanent competitive or 55-b/c status in the posted title and be eligible for lateral transfer; OR be eligible to transfer via Section 70.1, 70.4, or 52.6 of Civil Service Law. The transfer cannot be a second consecutive transfer resulting in an advancement of more than two salary grades.

Additional Comments • Please note that starting salary for this title reflects the Mid-Hudson adjustment. Posted Hiring Salary includes the sum of the statutory hiring rate for PS&T Grade 17 ($63,361) and Mid-Hudson adjustment ($2,000). Mid-Hudson adjustment is also added to the job rate ($80,655).

• If you are currently a New York State employee serving in a permanent competitive qualifying title and eligible for a transfer via Section 70.1 or 52.6 of the Civil Service Law, you may be appointed to this title as a permanent competitive employee.

• The assignment of PS&T employees will be made in accordance with any agreed upon Memorandum of Understanding between PEF and the local DDSO regarding the posting of job vacancies.

• All OPWDD employees must be eligible and maintain eligibility for full and unconditional participation in the Medicare and Medicaid programs. Continued employment will depend on maintaining eligibility.
