Minimum Qualifications Must currently be a permanent DDPS 1.

Must be active and reachable on the Civil Service eligible list for Developmental Disabilities Program Specialist 1.

Eligible for 70.1 transfer by way of current permanent status in a title deemed comparable to DDPS 1 by the Department of Civil Service (must also be within 2 salary grades of the targeted title).
Duties Description Provide Front Door Lead Services as follows:
* Oversee community support liaison who makes up the Westchester Office Community Support Services team.
* Provide technical support to these Liaisons and to the Westchester County Voluntary providers, families and local government.
* Assist certified residential opportunities processes involving Westchester county individuals and monitor residential referral requests.
* Outreach to local school district and transition meetings to access current transition population/age out needs.
* Monitor requests for assistive technology/environmental modifications.
* Oversee SEMP enrollments, be a contact and resource for Pathways Providers and NYS ACCES-VR. Assist in overseeing employment initiatives and attend statewide employment workshops/monthly meetings, (mostly via video conference).
* Oversee ISS requests on the Westchester Team (individual supports and services).
* Assist with Coordination of inter-district Transfers.
* Mobilize for and follow up to crisis situations that come to the Westchester office community support team.
* Oversee the Front Door Team for Westchester, including weekly meetings to prepare for QR, (Quality Review); to review the FD tracking sheet and address "bottlenecks" in the process accordingly; to present FD orientations. information related to the Westchester CS Services Team.
* Carry out hands-on FD functions as needed, (EAA's, DDP2's, etc.)
* Review CHOICES work queues.
* Network with Westchester Family Support Services providers.
