Duties Description Statewide Incident Oversight & Quality Assurance
· Oversee and provide real-time, statewide supervision of the incident management function, ensuring initial reports of serious incidents, abuse allegations, and deaths are thoroughly documented, correctly classified, and addressed with immediate protective measures and required notifications.
· Direct the review of completed investigations and supporting documentation; request additional evidence or investigative work as needed, and ensure accurate data entry into the Incident Report and Management Application (IRMA) and the Vulnerable Persons' Central Register (VPCR).
· Manage the tracking and review of Corrective Action Plans (CAPs), coordinate provider follow-up, and serve as the primary liaison to the NYS Justice Center for inquiries regarding investigations and CAPs.
· Oversee administrative and compliance workflows, including the processing of applications for the Mental Hygiene Law (MHL) background check and Statewide Central Register Database Check systems.
Inter-Agency Collaboration & Liaison Duties
· Collaborate with internal and external stakeholders --including the Office of Investigations and Internal Affairs (OIIA), the Office of Employee Relations, and local committees --to ensure a coordinated response to serious incidents and abuse allegations across state-operated and voluntary agencies.
· Coordinate with Division of Quality Improvement (DQI) units --specifically the Bureau of Program Certification (BPC) and Continuous Quality Improvement (CQI) --to share critical monitoring data and escalate significant untoward events requiring immediate intervention.
· Represent the Incident Management Unit (IMU) on local and statewide committees, workgroups, and initiatives focused on improving OPWDD incident processes.
Data Analysis, Reporting & Compliance
· Lead data analysis and reporting initiatives, monitoring IRMA data to identify systemic trends, generating Critical and Significant Event reports, and developing standardized trend reports for executive leadership and annual regulatory compliance.
· Provide guidance, technical assistance, and training to state-operated and voluntary providers on incident management protocols, ensuring compliance with 14 NYCRR Part 624 regulations and the Protection of People with Special Needs Act (PPSNA).
Administrative Operations & Process Management
· Establish, monitor, and refine internal unit processes and workflows to optimize efficiency, ensure regulatory compliance, and adapt to evolving unit needs.
· Maintain proficiency in unit databases and systems (Microsoft Office, IRMA, VPCR) to manage operations, streamline administrative processes, and resolve complex inquiries from stakeholders.
Staff Supervision & Leadership
· Supervise, direct, and develop unit staff; manage day-to-day performance, oversee time and attendance, and ensure timely completion of HR requirements (onboarding/offboarding, probationary reports, performance evaluations, and development plans).
· Support 24/7 unit operations by providing guidance and assistance to off-hours staff during standard and non-business hours to ensure continuous coverage and crisis response.
· Perform other duties and special projects related to incident management as assigned.

Minimum Qualifications Promotion:
Candidates must be reachable for appointment on the current Civil Service Eligible List.
Transfer:
Candidates must have one year of permanent competitive or 55-b/c status in the posted title and be eligible for lateral transfer; OR be eligible to transfer via Section 70.1, 70.4, or 52.6 of Civil Service Law. The transfer cannot be a second consecutive transfer resulting in an advancement of more than two salary grades.

• The assignment of PS&T employees will be made in accordance with any agreed upon Memorandum of Understanding between PEF and the local DDSO regarding the posting of job vacancies.
