Provisional Opportunity:
Candidates who meet the following qualifications may be provisionally appointed: Candidates must be a qualified employee of New York State and have had two years of permanent competitive or 55-b/55-c service as a Nurse Administrator 1, Nurse Administrator 1 (Psychiatric), Nurse Administrator 2, or Nurse Administrator 2 (Psychiatric); and must have a bachelor’s or master’s degree in Nursing, Nursing Adminstration, Health Care Adminstration, Public Health, Community Mental Health, or Health Administration.

Duties Description The Office of Mental Health (OMH) is seeking a qualified candidate to fill a Mental Hygiene Nursing Program Coordinator, M-1, position. Under the general direction of the Mental Health Program Manager 1 (Chief Executive Nursing Officer), the Mental Hygiene Nursing Program Coordinator will serve as a regional coordinator responsible for the coordination of all activities for the Nursing Home Enhanced Support Program in the assigned area of the State. The incumbent will work with Chief Nursing Officers, Community Mental Health Nurse (CMHN) and members of the Treatment Team, the Discharge Planning Team, the Psychiatric Center (PC) psychiatrists and staff, Skilled Nursing Facility (SNF) staff, Patient Resource Offices and Field Office staff on the collaboration, coordination and provision of enhanced supports to individuals discharged to skilled nursing facilities.

Duties to be performed include, but are not limited to the following:

• Identifies and collaborates with all stakeholders in problem solving and shared-decision making to address systems barriers, regional issues impacting the successful transition and sustainment of individuals in nursing homes.
• Negotiates, meets with and provides assistance to Nursing Home Directors and Administrators in resolving these barriers.
• In conjunction with the Chief Nursing Officer makes recommendations to policy, facility protocols and agreements with SNF related to transitioning of long stay patients to skilled nursing facilities.
• Provides ongoing nursing consultations including specific interventions for individuals referred from the PC to the SNF as requested by the nursing staff at the SNF.
• Collaborates with the SNF and PC staff in the development of the individualized care plan based upon familiarity caring for the patient at the PC and Coordinator’s competencies and knowledge of psychiatric nursing.
• Arranges and participates in weekly project planning and implementation meetings at each assigned facility.
• Attends and participates in long-stay/SNF discharge planning calls with State Operations staff in Central Office and with each facility.
• Oversees the agreement between the OMH PC and SNF to provide convalescent care and ensures OMH provides appropriate access to specialized psychiatric consultation services.
• Schedules and coordinates meetings with family members and significant others to address issues related to the referral and admission of individuals to SNFs.
• Ensures the requested enhanced nursing supports are provided to the SNF related to specific individuals discharged from OMH PCs to SNF.
• Collaborates with PC, SNF and CMHN to identify and secure other supports and personal items that might be needed or helpful to sustain the individual patient living in the nursing home.
• Develops, provides, and/or arranges for in-service education and training for Psychiatric Center nursing staff on best practices and interventions to better prepare long term individuals for placement in skilled nursing facilities.
• Provides targeted onsite staff trainings on evidenced based and best practices on the management of each individual referred and accepted from a PC to the SNF.
• Assists SNF staff to access OMH PC Psychiatric consultation services.
• Collects and compiles individual SNF tracking and status data for submission to the Office of Coordinated Nursing Services within the required timeframes.
• Collaborates with the PC, SNF and CMHN responsible for the submission of the monthly reports, data and the reporting on the status of individual patients at the designated intervals post discharge from the PC and admission to the SNF.
• Prepares metrics and reports for the legislature as required.
• Attends and participates in statewide meetings as necessary.

Additional Comments The incumbent of this position may be the regional coordinator for New York City and Long Island.

Posted Salary Range includes a Geographic Pay Differential of $12,871.

This position is eligible for the Downstate Adjustment of $3,026.
