Minimum Qualifications: Either A. licensed and currently registered by New York State as a Licensed Master Social Worker (LMSW) AND have one year of post NYS licensure professional social work experience; **
Or B. licensed and currently registered by New York State as a Licensed Clinical Social Worker (LCSW).

Duties Description PRIMARY DUTIES AND RESPONSIBILITIES:

• While working in an intensive treatment environment, provide treatment to developmentally disabled adults who may have been involved with the criminal justice system and ensure the coordination and delivery of social work services.
• Performs a wide variety of social work services for assigned individuals.
• Independently provides written social assessments and lends written and verbal input at treatment team meetings.
• Assesses individual’s social needs and resources available for optimal social functioning, then develops and implements goal directed programs to meet these needs.
• Monitors individual programs for continuity, consistency and progress as determined by the Interdisciplinary Treatment Team in accordance with ICF regulations.
• Provides one on one therapy, counseling sessions and/or provides classroom instruction to small groups of individuals during programming.
• Prepares and conducts in-services for in assessments, programs and other counseling interventions.
• Advocates for the needs of the individuals assigned.
• Assist in referrals to appropriate level of care.
• Provides updated information to Bureau of Revenue and Reimbursement and monitors individual financial situations and acts as advocate in determining funding eligibility.
• Obtains informed consent from individuals/families/legal guardians or Informed Consent Committee for any rights restriction.
• Provide administrative supervision to Licensed Master Social Workers 1, and to lower-level paraprofessional staff involved in case management and other treatment and program support tasks that do not require licensure.
• Teach therapeutic classes, as assigned.
• Other duties as assigned.
