Minimum Qualifications Minimum Qualifications:
For qualified employees of New York State: Must have had three months of permanent competitive as a
Senior Physical Therapist or Head Physical Therapist.

For all other qualified individuals: the candidate must be a licensed Physical Therapist in the State of
New York and have had post-licensure experience as follows: three years of work experience as a
Physical Therapist, one of which must have been in a supervisory capacity.

Duties Description PRIMARY DUTIES AND RESPONSIBILITIES:
The Head Physical Therapist plans and supervises physical therapy for individuals with developmental disabilities. Supervises a staff of physical therapy and adaptive equipment specialist personnel; plans and supervises training for staff; assigns staff to treatment teams and patients to individual therapists; approves individual treatment plans and group exercise plans; plans treatment for outpatient and state operated day care patients; works with community groups and agencies to ensure treatment of eligible groups.

• Observes, evaluates and documents individual's progress to determine response to physical therapy recommendations, interventions and treatment goals.
• Evaluates for and justifies Durable Medical Equipment as required.
• Develops individual specific methodologies for safe patient mobility for written staff instruction.
• Attends team meetings to represent the physical therapy components of the treatment plan as needed.
• Coordinates and integrates physical therapy recommendations with those of other disciplines.
• As member of a treatment team collaborates with other disciplines in identifying and writing
goals for individuals and reviews goals in conjunction with other staff.
• Instructs other physical therapy staff and service providers in therapy techniques and objectives.
• Comply with Sunmount operational policies and procedures.
• Participate as needed as a consultant with the community therapeutic dining team.
• Participates as a key member of the Safe Patient Handling committee and SPH Policy implementation.
• Works closely with Adaptive Equipment Specialist in maintaining, servicing, and ordering necessary equipment.
• Works as needed with contract Physical Therapy staff.
• Other Duties as Assigned.
