Minimum Qualifications Possession of a license and current registration as a registered professional nurse in New York State; and two years of post-licensure clinical nursing experience. One year of this experience must have been providing direct services to individuals diagnosed with mental illness, developmental disabilities, addiction, or substance use disorders in a community setting.

Substitution: a bachelor's degree in nursing may substitute for one year of general post-licensure clinical nursing experience; and a master's degree in nursing may substitute for two years of general post-licensure clinical nursing experience.

Duties Description POSITION DESCRIPTION:

The Nursing Home Liaison monitors OPWDD eligible individuals who reside in a nursing facility (SNF) and have been determined by the Level II PASRR* process to be eligible for a skilled nursing home level of care.

As a part of their role, the Nursing Home Liaison (NHL) will obtain regular updates regarding the individual’s medical and overall wellbeing. Additionally, the NHL will ensure that individuals placed in nursing home facilities are residing in the most appropriate, least restrictive community setting that meets their needs. Additionally, the NHL will collaborate with involved stakeholders such as the CCO, Regional Field Office units, and OPWDD licensed providers.

PRIMARY DUTIES AND RESPONSIBILITIES INCLUDE BUT ARE NOT LIMITED TO:

• NHL ensures each nursing home is providing the monthly status report that identifies the medical/health, wellness, desires, and discharge readiness status for each individual being served in the SNF.

• Bimonthly (every 2 months) standing meetings should be held by TEAMS video meeting (best practice) or phone call to review individuals in the NH facility and will be used to verify new admissions, discuss any notable changes to medical condition/functioning including hospitalizations, identify those ready for discharge (or discharged), and verify deaths. After each meeting, the NHL will update the R4 NH tracker with the information for individuals discussed.

• Nursing home will notify NHL of any significant events (discharges, deaths, other). This information will be tracked and communication with other units will occur as necessary.

• Each NHL will be assigned a group of NHs they’ll support and will be the point of contact for those facilities. The NHLs will cover for each other when the other is absent.

o NHL will develop a relationship with each of the nursing home contacts and be a informational/linkage resource and bridge between the RFO and NH facility

o Provide OPWDD systems education and information regarding supports and services including access to housing, crisis, front door, eligibility, etc.

o Identify those who may be ready for transition back to a more community inclusive setting and coordinate between stakeholders for purposes of discharge planning.

o For those being served by a certified (IRA or ICF) program, coordinate with the agency and CCO to ensure a smooth transition occurs. Address due process/633.12 issues as they arise, coordinating with QMTA as necessary.

o As necessary, NHL will participate in discharge planning meetings and connect with the individual/family to answer questions and connect them with the front door (and CCO when necessary) to assess service and transition needs.

• Identify those who may be appropriate for a specialty hospital admission and help facilitate an application when applicable.

• Identify in the NH tracker Willowbrook class members.

o Obtain monthly updates regarding each individual including their level of functioning, medical needs/changes/condition, and readiness for discharge.

o Provide updates to the assigned RFO WB liaison, flagging when there are changes to individual’s medical condition (decline including hospitalizations/improvement) and readiness for discharge.

Posted Hiring Salary includes the sum of the statutory hiring rate ($69,934) for PS&T Grade 20, Geographic Pay Differential ($35,000), and Location Pay ($3,026). Maximum salary includes the sum of Job Rate ($88,721), Geographic Pay Differential ($35,000), and Location Pay ($3,026).
