Minimum Qualifications Permanent Transfer Candidates: Current NYS Department of Health employee with permanent or contingent-permanent status as a Health Systems Specialist 2 (Social Work), G-20 OR a current NYS employee with one year or more of permanent or contingent-permanent service in a position G-18 or higher, and eligible for 70.4 transfer (must pass exam by meeting provisional qualifications below).
Non Competitive Minimum Qualifications: Possession of New York State license and current registration as a licensed master social worker (LMSW), or licensed clinical social worker; and two years of post-LMSW license experience providing clinical services in a licensed healthcare facility or program, or utilizing clinical knowledge in the review and oversight of clinical services for quality assurance or reimbursement purposes.
Preferred Qualifications: Experience with providing case management services in a long-term care facility or care management along the long-term care continuum. A strong dedication and desire to impact the quality of life for the aged and aging. Excellent communication skills, both oral and written. Federal Surveyor Minimum Quality Test certification and investigatory experience is a plus.
Duties Description With an eye toward continuous quality improvement, serve the Metropolitan Regional Office team as the premier social work services expert and in this capacity, assess State and Federal regulatory compliance through onsite surveys conducted as part of an interdisciplinary team or independently depending on the nature of the assessment. This will require that the incumbent meet with social work teams at adult care facilities and conduct interviews of residents to assess overall compliance as needed to drive quality of life improvements in such facilities. In addition, the incumbent will prepare well-written reports related to their findings, participate in interdisciplinary reviews as needed, and provide constant feedback about necessary improvements to impact the suitability of aged and aging services. In this team-oriented environment, the incumbent may be asked to participate in other program-related activities as needed.
Additional Comments This position requires travel at times to areas unserved by public transit, including overnight travel and travel throughout the lower Hudson Valley. The incumbent must pass the Surveyor Minimum Qualifications Certification Test (SMQT) within 1 year of appointment.
Plus a $3,087 downstate pay.

For new State employees appointed to graded positions, the annual salary is the hiring rate (beginning of the Salary Range) of the position. Promotion salaries are calculated by the NYS Office of the State Comptroller in accordance with NYS Civil Service Law, OSC Payroll rules and regulations and negotiated union contracts.
