Minimum Qualifications Permanent Qualifications: Candidates must be licensed and currently a registered professional nurse in New York State; and two years of post-licensure clinical nursing experience. One year of this experience must have been providing direct services to individuals diagnosed with mental illness, developmental disabilities, addiction, or substance use disorders in a community setting.
Substitution: a bachelor’s degree in nursing may substitute for one year of general post-licensure clinical nursing experience; and a master’s degree in nursing may substitute for two years of general post-licensure clinical nursing experience.

Transfer Qualifications: Candidates must currently be a permanent Registered Nurse 2 or be eligible for transfer under Section 70.1 of the Civil Service Law

Duties Description The Registered Nurse 2 (Community) position will provide administrative oversight to assigned Family Care Homes. The Family Care RN 2 will also work within the framework of a treatment team providing nursing service to individuals diagnosed with developmental disabilities, and their caretakers, the Family Care Providers. The Family Care RN 2 visits the individuals in their home or in the community to make recommendations for emergency or continuing care, suggest alternative treatment plans, and teach the Family Care Provider (FCP) methods of dealing with emergencies. They also ensure that the health care needs of the individuals are provided and work with appropriated community agencies as necessary and ensure a coordinated team treatment plan. The Family Care RN 2 will be assigned to Family Care homes located in Dutchess County and may provide coverage in other counties. The incumbent will be based out of the Poughkeepsie office.
Duties include but are not limited to:
• Ensures compliance with the Taconic DDSOO Policy/Procedures and regulatory requirements.
• Provides clinical oversight of the individuals health, coordinates the individual’s health care services, conducts on site announced and unannounced visits, develops plan of nursing service, provide nursing care, assist with medical appointments, provides guidance, and makes recommendations based on the individuals medical needs.
• Acts as a liaison between the Individual, Family Care Provider, involved families, and other members of the treatment team.
• Communicate the individual’s health condition/care to treatment team.
• Evaluates the individual’s health care needs and treatment plan for effectiveness and make recommendations for emergency and/or continuing care.
• Communicates and coordinate care with community agencies including day programs to review treatment plan and adjusts, as necessary.
• Participates in assessing the Family Care Provider’s skills and abilities in addressing the needs of the individuals.
• Provides guidance and training to the Family Care Providers in dealing with emergencies/routine medical care.
• Provides training, education and support to the Providers and the individuals regarding health care issues and preventative health strategies.
• Participates in the assessment of the home’s physical environment and develops recommendations such as environmental modifications as appropriate to meet the individuals needs based on the environmental assessment.

Additional Comments This title is part of the New York Hiring for Emergency Limited Placement Statewide Program (NY HELPS). HELPS Program titles may be filled via a non-competitive appointment. This means that you do not need to take an exam to qualify, but you do need to meet the minimum qualifications of the title.

If you are hired under the HELPS Program, you will be first be appointed as a temporary employee. In May 2024, if you are performing successfully in the position, your appointment will be changed from temporary to permanent non-competitive. Once you are in permanent status, your official probationary period will begin and you may take any promotion exam for which you are qualified.

At a future date (within one year of permanent appointment), employees hired under NY HELPS are expected to have their permanent non-competitive employment status converted to permanent competitive status. You will not have to take an exam to gain permanent competitive status.

• The assignment of PS&T employees will be made in accordance with any agreed upon Memorandum of Understanding between PEF and the local DDSO regarding the posting of job vacancies.
