Duties Description Provides paraprofessional rehabilitation services to individuals in psychosocial support, vocational rehabilitation, competency training, and residential services program areas.
• Interviews and gathers individuals’ information; assists in conducting assessments to determine individuals’ potential and existing capabilities and interests; and in developing and implementing rehabilitation plans, including goals and objectives.
• Assists in planning and conducting group rehabilitation activities, and in developing group protocols and curriculum.
• Observes and reports individuals’ participation, response, and behavior; and may recommend modifications to rehabilitation plans based on observations.
• Participates in treatment team meetings; and assists in determining individuals’ progress and rehabilitation needs.
• Maintains records and documentation in accordance with program standards and requirements.
• May monitor and maintain supply inventories; and schedule and arrange for rehabilitation activities and special events.

The Community Pre-Vocational (CPV) Team Member is responsible for facilitating the mission of the CPV program; provides outlets for individuals to develop and improve soft skills geared towards employability and to assist with administrative duties including training of staff, facilitating Job Readiness Training Classes, completing annual assessments, program development, Documentation Standards and other CPV related activities.

Develop training curriculums, activities, and experiences for individuals via in-person Job Readiness Training Classes. Facilitate Job Readiness Training classes to individuals to prepare for successful transition from volunteering to employment. Model and promote soft skills and work-related etiquette.

Demonstrate understanding of the OPWDD Community Prevocational regulations, Administrative Memorandum, audit protocols and documentation standards in order review documentation and mentor Job Coaches. Attend Life Plan meetings and assist with the development of Staff Action Plans. Review documentation and facilitate timely corrections. Complete Annual Assessments.
Assist CPV Coordinator in the development, expansion, and implementation of community-based vocational opportunities. Make connections for volunteer experiences. Conduct volunteer site satisfaction surveys. Train staff to assist meeting the expectations of volunteer sites and promotion of soft skill development with individuals. Ensure CPV activities are provided during staffing shortages by job coaching in the community when necessary. Travel required – 20%.

Minimum Qualifications Rehab Assistant 1 Promotion: one year of service as a Direct Support Assistant, Mental Health Therapy Aide, or Licensed Practical Nurse and reachable for appointment on the current Civil Service Eligible List for this title.

Rehab Assistant 1 Open Competitive: One year of experience assisting individuals with daily living skills and habits; taking care of their personal needs; and ensuring a comfortable home environment.

Rehab Assistant 2 Promotion: two years of service as a Rehabilitation Assistant 1. *

Transfer:
Candidates must have one year of permanent competitive or 55-b/c status in the posted title and be eligible for lateral transfer; OR be eligible to transfer via Section 70.1, 70.4, or 52.6 of Civil Service Law. The transfer cannot be a second consecutive transfer resulting in an advancement of more than two salary grades.

Additional Comments This title is part of the New York Hiring for Emergency Limited Placement Statewide Program (NY HELPS). HELPS Program titles may be filled via a non-competitive appointment. This means that you do not need to take an exam to qualify, but you do need to meet the minimum qualifications of the title.

At a future date (within one year of permanent appointment), employees hired under NY HELPS are expected to have their permanent non-competitive employment status converted to permanent competitive status. You will not have to take an exam to gain permanent competitive status.

• If you are currently a New York State employee serving in a permanent competitive qualifying title and eligible for a transfer via Section 70.1 of the Civil Service Law, you may be appointed to this title as a permanent competitive employee.

• The assignment of PS&T employees will be made in accordance with any agreed upon Memorandum of Understanding between PEF and the local DDSO regarding the posting of job vacancies.
