Duties Description The Quality & Risk Management Specialist will play a critical role in Quality Management and Risk Management functions within the facility. Responsibilities include investigating significant incidents, addressing patient and family complaints, collecting and analyzing data, and ensuring regulatory compliance. The ideal candidate will have excellent communication and organizational skills, with a proactive approach to promoting patient safety and continuous improvement.
Risk Management:
• Conduct and oversee all aspects of investigations as per Office of Mental Health (OMH) policies and procedures, including but not limited to:
o Interview staff and gather relevant documentation.
o Coordinate Justice Center interviews with facility leadership for patients and staff.
o Complete comprehensive reports on significant incident investigations within 30 days.
o Identify incident causes, patterns, and trends to inform risk reduction strategies.
• Conduct risk management orientation and related training sessions for staff.
• Participate in environmental rounds to identify and mitigate potential safety risks.
• Review incident and restraint/seclusion forms, ensuring accurate entry into the NIMRS database.
• Serve as the liaison between the facility, OMH field office, Central Office, and the NYS Justice Center.
• On-Call Availability: Be available during off-shifts and weekends to address reportable incidents as needed.
Quality Management:
• Conduct FMEA and Root Cause Analysis, providing insights based on identified trends.
• Participate in preparation for The Joint Commission (TJC) and Office of Quality Improvement (OQI) surveys.
• Complete standards compliance and survey-related activities to meet regulatory requirements.
• Facilitate TJC and OQI surveys when surveyors are on site.
• Develop and execute plans of correction (POCAs) as needed.
• Review and analyze facility performance data, producing reports and recommending programmatic improvements.
• Present reports, data and trends, and the like to Cabinet, and various committees.
Minimum Qualifications Candidates from outside or within State Service can be considered for hire under the Hiring for Emergency Limited Placement – Statewide (NY HELPS) program if they are licensed and currently registered by New York State as a Licensed Master Social Worker (LMSW).
OR
Candidates within State Service can be considered transfer to a competitive class position if they have one year of permanent competitive or 55-b/55-c service in a title eligible for transfer under Section 70.1 of the Civil Service
Qualifications:
• Strong verbal and written communication skills, with the ability to engage e effectively with diverse audiences including administration, clinicians, direct care, and support staff.
• Excellent organizational and time management skills.
• Proficiency in computer systems and database management.
• A proactive, analytical mindset with attention to detail.
Preferred Experience:
• Familiarity with OMH policies, TJC standards, and risk management principles.
• Previous experience in healthcare quality management or risk management roles is a plus.

This position is eligible for a Downstate Adjustment of $4,000.
