Duties Description The Clinical Physician 2 Internist provides comprehensive medical management and oversight for patients/residents in the acute rehabilitation and subacute programs. This physician plays a key leadership role as Medical Director of the hospital's Skilled Nursing Facility (SNF) and Transitional Care Unit (TCU), ensuring high-quality, patient/resident–centered care through collaboration with physicians, nursing, therapy, respiratory, case management/social workers and ancillary departments. The role includes providing direct patient care, acute and subacute medical consultations and administrative leadership for subacute operation, performance improvement and regulatory compliance.
Minimum Qualifications Transfers: Current New York State employee with one or more years of permanent, contingent-permanent, or 55 b/c service
in a position allocated to Grade 34 or above, and that is eligible for transfer under Section 70.1 of the Civil Service Law to Clinical Physician 2.
Open Competitive HELPS Minimum Qualifications:
Possession of a license and current registration to practice medicine in New York State and two years of post-licensure medical experience.
Promotion Provisional Appointment: In the absence of an eligible list with three or more candidates willing to accept a position, a temporary provisional appointment may be considered. For provisional appointment, candidates must meet one of the minimum qualifications: Be a current NYS employee with one year of permanent competitive or 55-b/55-c service as a Clinical Physician 1 and possession of a license and current registration to practice medicine in New York State.
Preferred Qualifications: Doctor of Medicine (MD) or doctor of Osteopathy (DO) from an accredited institution. Board Certified or Board Eligible in Internal Medicine (ABIM or AOBIM). Active and unrestricted NY State medical license. Current DEA registration, Basic Life Support (BLS), and Advance Cardiac Life Support (ACLS) certifications. 2-3 years of post-residency experience in inpatient acute rehabilitation, skilled nursing facility, or hospitalist medicine. Proven experience managing medically complex patients (neurological, cardiac, pulmonary, post-surgical, and infectious disease conditions). Demonstrated ability to work effectively in interdisciplinary care teams focused on patient recovery and functional improvement. Experience in care transitions, discharge planning, and readmission prevention. Strong knowledge of rehabilitation medicine principles and post-acute medical management. Proficient in chronic disease management, infection control, and polypharmacy oversight. Skilled in conducting comprehensive medical evaluations and coordinating multidisciplinary treatment plans. Ability to participate in family conferences and patient/resident family education regarding goals of care. Excellent interpersonal and communication skills to foster collaboration across disciplines. Commitment to patient-centered care and high-quality clinical outcomes. Participation in quality improvement, utilization review, and clinical documentation integrity initiatives. Willingness to mentor mid-level providers, residents, or students. Familiar with electronic health record (EHR) systems. Engagement in evidence-based practice and performance improvement projects. Interest in program development and rehabilitation outcome research.

Non-Competitive Promotion: If there are three or fewer qualified candidates interested in the appointment to this position, a permanent appointment may be made pursuant to Section 52.7 of the Civil Service Law. To be eligible, candidates must be an employee of the Department of Health Helen Hayes Hospital and have one year of permanent competitive service as a Clinical Physician 1, and possession of a license and current registration to practice medicine in New York State.
Posted salary range includes statutory hiring rate at the job rate for PS&T Grade 36 ($247,006 to $247,006), plus an additional geographic pay differential of $40,000 and downstate adjustment of $4,000.
